LF托槽非拔牙矫治Ⅰ度牙列拥挤的临床研究

A Clinical Study of LF Brackets in the Class One Crowding by the Nonextraction Treatment

目的分析研究使用LF托槽与传统MBT托槽非拔牙矫治Ⅰ度牙列拥挤时患者牙弓、颌骨及面部软组织的改变。方法将32例牙列Ⅰ度拥挤患者随机分为两组,每组各16例,分别使用LF托槽和MBT托槽。矫治前后患者拍摄头颅侧位片及制取记存模型,定点、测量12项测量项目,比较分析测量结果及治疗时间。结果治疗后两组患者软组织侧貌及牙弓宽度均有一定程度增加(P<0.05)。治疗前后差值比较时,LF托槽组磨牙间宽度改变值明显大于MBT托槽组(P<0.01);MBT托槽组软组织侧貌和尖牙间牙弓宽度改变值明显大于LF托槽组(P<0.01)。LF托槽组治疗时间少于MBT托槽组(P<0.01)。结论对于Ⅰ度拥挤非拔牙矫治患者,MBT托槽组主要靠前牙唇倾和尖牙弓的扩宽来解除拥挤,而LF托槽组主要靠扩宽磨牙段牙弓宽度来获得排齐间隙,并且使用LF托槽治疗时间明显短于使用MBT托槽,有助于提高正畸医师临床工作效率。

Objective To analyze the changes in the class one crowding of the patients＇ dental arch,jaw and facial soft tissue with LF brackets by the nonextraction treatment. Methods Thirty two cases of the class one crowding patients were randomly divided into LF brackets and MBT brackets,with 16 cases in each group. The dental casts and cephalometric X-Ray were taken before and after treatment,12 measurements were traced and analyzed,and a comparative analysis of the result of measurement and the time of treatment was also made. Results The soft tissue profile and dental arch width of these two groups statistically significantly increased after treatment（ P〈0. 05）. A comparison of the molars width before and after treatment indicated that LF brackets group increased significantly more than MBT brackets group（ P〈0. 05）. The changes of soft tissue profile and canines width in MBT group increased significantly more than LF brackets group（ P〈0. 05）. The time of treatment in LF brackets group were significantly shorter than MBT brackets group（ P〈0. 05）. Conclusion In the class one crowding by the nonextraction treatment,the MBT brackets group relieved crowding through the labial inclination of incisors and the canines widening,while the LF brackets relieved crowding through the molars widening. In addition,the time of treatment in LF brackets group were significantly shorter than MBT brackets group. Therefore,it＇s helpful for the LF brackets to improve the clinical efficiency of orthodontics.
