摘要 目的探讨应用CT诊断新生儿窒息后脑损伤的临床应用价值。方法 110例窒息后脑损伤患儿,随机分为观察组和对照组,每组55例。观察组患儿采用日本东芝公司生产的多排螺旋CT技术进行诊断,对照组患儿采取常规B超进行诊断。观察两组诊断结果并做好观察记录,分析观察组CT诊断分度与颅内损伤情况之间的关系,比较两组患儿应用相应诊断技术后的病情诊断准确率。结果观察组患儿经螺旋CT扫描诊断后,按照缺氧缺血性脑病的临床分级标准进行评估,其中12例为重度,19例为中度,24例为轻度。观察组患儿病情诊断的准确率为87.27%,对照组患儿病情诊断的准确率为52.73%,观察组患儿病情诊断的准确率明显高于对照组,差异具有统计学意义(P<0.05)。结论利用多排螺旋CT对患儿进行诊断,能直观、客观、快捷、敏感地反映脑损伤的范围、程度,有利于观察病情的发展,其具有高敏感性、高特异性,可提供良好的预测值,协助患儿诊断治疗,改善患儿预后,具有非常好的临床指导意义,值得广泛推广和应用。 Objective To discuss the clinical application value of CT diagnosis for brain injury after neonatal asphyxia. Methods A total of 110 children with brain injury after neonatal asphyxia were randomly divided into observation group and control group, with 55 cases in each group. The observation group was diagnosed by multi row spiral CT technology produced by the Japanese Toshiba Co., and the control group was diagnosed by conventional B-mode ultrasound. The diagnostic results of the two groups was observed and recorded. The relationship between the CT diagnostic degree and intracranial injury in the observation group, and the diagnostic accuracy of the two groups after the corresponding diagnostic techniques was compared. Results After the diagnosis of spiral CT scan, the observation group was evaluated by clinical grading standard of hypoxic ischemic encephalopathy, of which 12 cases were severe, 19 cases were moderate, and 24 cases were mild. The observation group had accuracy of disease diagnosis as 87.27%, while the control group had accuracy of disease diagnosis as 52.73%. The observation group had obviously higher accuracy of disease diagnosis than the control group, and the difference was statistically significant(P〈0.05). Conclusion The multi slice spiral CT can directly, objectively, quickly and sensitively reflect the scope and degree of brain injury, and observe the condition of the development. It has high specificity and sensibility, and can provide a good predictive value, help children with the diagnosis and treatment of children, and it improves the prognosis of children. It is of great significance in clinical guidance and is worthy of extensive popularization and application.
