盐酸右美托咪定与咪唑安定在机械通气中镇静作用对比分析 被引量:1

摘要 目的研究盐酸右美托咪定与咪唑安定在机械通气中的镇静作用。方法选取我院2016年6月1日~2017年5月31日收治的60例机械通气患者,将其计算机随机分为两组,每组30例。其中,对照组患者使用咪唑安定,观察组采用盐酸右美托咪定。比较两组患者的镇静效果、达到镇静时间、停药后苏醒时间、机械通气时间、停药后拔管时间、住院时间、相关指标、不良反应发生情况。结果治疗后,观察组患者的Ramsay评分低于对照组,达到镇静时间、停药后苏醒时间、停药后拔管时间、住院时间均短于对照组(P<0.05)。观察组患者镇静后的血压、心率和呼吸频率与镇静前比较,差异无统计学意义(P>0.05),与对照组镇静后比较,差异有统计学意义(P<0.05)。两组的机械通气时间差异无统计学意义(P>0.05)。观察组患者的不良反应发生率低于对照组(P<0.05)。结论在机械通气中采用盐酸右美托咪定具有较强的镇静效果和较高的安全性,比咪唑安定更具优势,适合被推广应用在临床中。 Objective To study the sedative effect of Dexmedetomidine Hydrochloride or Midazolam in mechanical ventilation.Methods 60 patients with mechanical ventilation from June 1,2016 to May 31,2017 treated in our hospital were selected and they were divided into the two group in random by computer and there were 30 cases in each group. Patients in the control group were given Midazolam,while patients in the observation group were given Dexmedetomi- dine Hydrochloride.The sedative effect,time to take sedation,awakening time after drug discontinuance,mechanical venti- lation time,extubation time after drug discontinuance,hospital stay,related indexes,occurrence of adverse reaction were compared in patients between the two groups.Results After sedation,Ramsay score in patients of the observation group was lower than that of the control group,and time to take sedation,awakening time after drug discontinuance,extubation time after drug discontinuance,and hospital stay were shorter than those in the control group (P〈0.05).The blood pres- sure,heart rate,and respiratory frequency after sedation were not displayed statistical differences compared with before in the observation group (P〉0.O5).There were statistical significance in these indicators between the observation group and control group after sedation (P〈0.05).There was no statistical difference of mechanical ventilation time between in- ter-group comparison (P〉0.05).The incidence of adverse reaction in the observation group was lower than that in the control group (P〈0.05).Conclusion Compared with Midazolam,application of Dexmedetomidine Hydrochloride in me- chanical ventilation exerts a more powerful sedative effect and higher safety,which is applicable for clinical promotion and application.
