重症早期预警护理策略在急诊小儿手足口病中的应用

Clinical application of early intensive prevention nursing interventions in the treatment of hand foot and mouth disease

目的研究重症早期预警护理策略在急诊小儿手足口病中的应用效果。方法选择我院急诊科2016年7月~2017年6月收治的39例小儿手足口病患儿,纳入观察组,实施重症早期预警护理干预;急诊科于2015年8月~2016年6月收治的39例小儿手足口病患儿,纳入对照组,实施常规护理服务,评价两组患儿症状消退用时、重型发生率、家长对护理质量评分。结果护理干预后,观察组患儿疾病发展分型包括普通型38例(97.44%)、重型1例(2.56%);均优于对照组34例(87.18%)、5例(12.82%)。观察组患儿家长对病情观察、重症预警、降温护理、生活护理等指标评分均高于对照组(t=42.401、47.602、33.317、31.685,P<0.05)。观察组患儿症状消退时间、发热消退时间、住院时间均短于对照组,差异有统计学意义(t=7.853、11.024、5.263,P<0.05)。结论实施重症早期预警护理干预可有效阻止手足口病患儿病情恶化可能性,减少重型手足口病发生率,促进症状消退,提高护理质量。

Objective To observe the clinical application of early intensive prevention nursing interventions in the treat- ment of hand foot and mouth disease （HFMD）.Methods 39 HFMD children treated from July 2016 to June 2017 in our emergency department were selected as observation group,the early intensive prevention nursing interventions were ap- plied.39 HFMD children treated in our emergency department from August 2015 to June 2016 were selected as control group,the conventional nursing care was applied.The disappearance time of clinical symptoms,incidence rate of severe HFMD and the family member＇s scores on the nursing quality for two groups were evaluated.Results After interven- tions,the observation group had 38 cases of general type of HFMD （97.44%） and one severe HFMD case （2.56%）;the control group had 34 cases of general type of HFMD （87.18%） and 5 severe HFMD cases （12.82%）.The evaluation scores of clinical observation,prevention of severe HFMD cases, cooling nursing care and life nursing care for observa- tion group were significantly higher than control group （t=42.401,47.602,33.317,31.685,P〈0.05）.The disappearance time of clinical symptoms and fever and hospitalization time for observation group were significantly shorter than control group,the differences were statistically significant （t=7.853,11.O24,5.263,P〈0.O5）.Conclusion The early intensive pre- vention nursing interventions can efficiently avoid the exacerbation of HFMD,reduce the incidence rate of severe HFMD,shorten the disappearance time of clinical symptoms and improve the nursing quality.
