基于跨理论模型的社区家庭访视护理对老年高血压患者服药遵从行为的影响

The effect of community family visit nursing on the compliance behavior of elderly hypertension patients based on cross-theoretical model

目的探讨基于跨理论模型的社区家庭访视护理对老年高血压患者服药遵从行为的影响。方法选取我院2016年1~12月收治的高血压患者52例,按照患者就诊顺序分成观察组与对照组,每组各26例。对照组采用常规家庭访视护理干预,观察组应用基于跨理论模型的社区家庭访视护理干预,观察两组患者服药遵从行为情况及血压变化。结果观察组患者在自行停止服药、自行减少服药次数、自行减少服药剂量、遵医嘱定时服药、遵医嘱定量服药行为方面及总评分与对照组比较,差异有统计学差异(P<0.05);在服药依从性的行为改变阶段中,观察组患者行动阶段比例明显高于对照组(P<0.05);观察组患者往前跨越1、2阶段比例明显高于对照组,观察组服药依从性的行为改变阶段跨度明显优于对照组,差异有统计学意义(P<0.05);经护理,观察组在4、8、12周时的收缩压均低于对照组,差异有统计学意义(P<0.05)。结论基于跨理论模型的社区家庭访视护理干预对于老年高血压患者的服药遵从行为具有明显改善作用,可提高患者依从性,有效控制血压,临床使用价值高。

Objective To explore the effect of community home visiting nursing based on cross theoretical model on medication compliance behavior of elderly patients with hypertension.Methods 52 cases of hypertension in our hospital from January 2016 to December were selected and divided into observation group and control group according to the order of patients,26 cases in each group.The control group was treated with routine home visiting nursing intervention, and the observation group was treated with community based home visiting nursing intervention based on the cross theoretical model.The compliance behavior and blood pressure changes of the two groups were observed.Results The patients in the observation group to stop medication,to reduce the frequency of administration,to reduce the dosage,the doctor prescribed medication,medication timing quantitative behavior and total score were compared with the control group,the differences were statistically significant （P〈0.05）.The change of compliance behavior stage of the patients in the observation group were significantly higher than the proportion of the action stage inXthe control group （P〈0.05）.The patients in the observation group were 1 and 2 stage forward across the proportion were significantly higher than the control group,the observation group compliance behavior change stage span was significantly better than the control group,the differences were statistically significant （P〈0.05）.After nursing,observation group in 4,8 and 12 weeks the systolic blood pressure control were lower than the control group,the differences were statistically significant （P〈0.05）. Conclusion The transtheoretical model of community family visit nursing intervention on medication compliance of el- derly patients with hypertension has obvious improvement effect on,can improve patient compliance,effective control of blood pressure,clinical use of high value.
