宫颈上皮内瘤变证候特征与宫颈免疫因子TNF-α相关性临床研究

Clinical Study on the Relativity between Reof Cervical Intraepithelial Neoplasia Syndrome and Cervical Immune Factor

目的:探讨宫颈上皮内瘤变证候特征和宫颈免疫因子TNF-α的内在联系,为临床治疗宫颈上皮内瘤变(Cervical intraepithelial neoplasia,CIN)提供依据。方法:本次研究采取回顾性分析的方法,选取2015年2月到2017年2月在本院行宫颈上皮内瘤变治疗的患者86例,应用免疫组织化学SP法检测患者宫颈组织标本中TNF-α的阳性表达,通过图象分析软件对TNF-α的阳性表达进行半定量分析。结果:86例CIN患者中,其中CIN I有34例,CIN II有28例,CIN III有24例,CIN I患者多属于脾虚湿盛型,CIN II患者多属于湿热内蕴型,CIN III患者也多属于湿热内蕴型;TNF-α在三种级别的患者的组织中表达的IOD值分别为4.097±1.501、6.286±1.428、7.601±1.470,差异具有统计学意义(P<0.05);不同级别的CIN病变中TNF-α的表达强度与宫颈病变程度呈正相关关系;TNF-αIOD值在各种类型的CIN中有明显的趋势,其中脾虚湿盛型值最高,肝郁气滞型值最低,差异具有统计学意义(P<0.05)。结论:TNF-αIOD值与宫颈病变程度呈正相关关系,宫颈免疫因子TNF-α的表达程度对中医分型有一定的参考价值,可以为阻断宫颈上皮内瘤变提供依据。

Objective： To study the intrinsic relationship between cervical intraepithelial neoplasia syndrome and cervical im- mune factor TNF-α, and to provide the basis for clinical treatment of cervical intraepithelial neoplasia （ cervical intraepithelial neoplasia, CIN）. Methods： A retrospective analysis was conducted in this study. 86patients with cervical intraepithelial neoplasia from February 2015to February 2017were selected. Immunohistochemical SP method was used to detect the expression of TNF- α tpositive expression, semi-quantitative analysis of TNF-α positive expression by image analysis software. Results： Among the 86cases of CIN, 34cases were CIN I, 28cases were CIN II, 24cases were CIN III, CIN I patients were mostly spleen dampness type, CIN II patients were mostly damp-heat type, CIN III patients. The IOD valuesof TNF-α were 4. 097±1. 501, 6. 286± 1. 428and 7. 601±1. 470, respectively, and the difference was statistically significant （P〈0. 05 ）. There was a positive correlation between the expression of TNF-α and the degree of cervical lesions in the CIN lesions. The TNF-odOD values showed a significant trend in various types of CIN, with the highest spleen dampness value and the lowest, the difference was statistically significant （P〈0. 05）. Conclusion： The level of TNF-α IOD is positively correlated with the degree of cervical lesions. The expression of TNF-α is of great reference value to the classification of cervical intraepithelial neoplasia.
