经颅多普勒检测脑动脉狭窄与脑血管造影磁共振血管成像CT血管造影的对比研究 被引量:7

摘要 目的比较经颅多普勒(TCD)与脑血管造影(DSA)、磁共振血管成像(MRA)、CT血管造影(CTA)对缺血性脑血管病颅内动脉狭窄或闭塞诊断的检出率。方法 137例经TCD筛查有脑动脉狭窄患者,之后行DSA(25例)或MRA(52例)或CTA(60例)检测。分析比较TCD与DSA、MRA及CTA检测脑动脉狭窄的检出率。结果 TCD与DSA对脑动脉狭窄的检出率比较,差异无统计学意义(P>0.05),而MRA、CTA对脑血管的检出率明显低于TCD,差异有统计学意义(P<0.05)。结论 TCD对脑动脉狭窄的检出率与DSA相近,而高于MRA和CTA,说明TCD价廉,操作方便,可靠性强,是筛查脑动脉狭窄的很好工具。 Objective To compare the accuracy of transcmnial Doppler (TCD) and cerebral angiography (DSA), magnetic resonance angiography (MRA) and CT angiography (CTA) in the diagnosis of intracranial arterial stenosis or occlusion of ischemic cerebrovascular disease. Methods One hundred and thirty-seven patients with cerebral arterial stenosis were examined by TCD, followed by DSA (25 cases) or MRA (52 cases) or CTA (60 cases). Analysis of TCD and DSA, MRA and CTA detection of cerebral artery stenosis detection rate. Results There was no significant differ-ence in the detection rate of cerebral artery stenosis between TCD and DSA (P〉0.05). The positive rate of MRA and CTA was significantly lower than that of TCD (P〈0.05). Conclusion The detection rate of TCD on cerebral artery stenosis is similar to that of DSA, but higher than that of MRA and CTA, which indicates that TCD is inexpensive, easy to operate and reliable. It is a good tool for screening cerebral artery stenosis.
