子宫内膜癌与卵巢癌患者化疗前后肿瘤血管阻力指数和CA125水平比较

Comparison of Tumor Vascular Resistance Index and CA125 Level in Patients with Endometrial Cancer and Ovarian Cancer before and after Chemotherapy

目的探讨子宫内膜癌与卵巢癌患者化疗前后血清CA125水平和子宫动脉阻力指数(RI)变化,旨在分析上述指标在判断不同类型肿瘤化疗后转归的价值。方法随机选取我院子宫内膜癌患者40例,另选取卵巢癌患者30例,所有患者均采用TP化疗方案,化疗前和化疗结束后检测患者子宫动脉阻力指数和血CA125水平。结果化疗前,两组RI均<0.5,化疗后子宫内膜癌组RI升高较明显,明显高于卵巢癌组,组间比较差异具有统计学意义(P<0.05);化疗前两组CA125水平较高,但差异无统计学意义(P>0.05),但化疗后子宫内膜癌组CA125下降更为明显,与卵巢癌组比较差异具有统计学意义(P<0.05);在子宫内膜癌组,随着RI升高,CA125逐渐下降,在肿瘤区血流RI与CA125水平具有良好相关性(r=-0.75,P=0.001),但在卵巢癌组两者未见明显相关性(P>0.05)。结论化疗前、后子宫及其附件恶性肿瘤中血流谱存在显著性差异,而子宫动脉阻力指数和CA125更有助于判断子宫内膜癌患者化疗的疗效,具有一定的临床价值。

Objective To study the change of uterine artery resistance index（ RI） and serum CA125 level in patients with endometrial cancer and ovarian cancer before and after chemotherapy to analyze the value of these indexes in judging the outcome of different types of tumor after chemotherapy. Methods 40 cases with endometrial carcinoma and 30 cases with ovarian cancer treated in our hospital were randomly selected. All patients were treated with TP chemotherapy. The uterine arterial resistance index and blood CA125 level were detected before chemotherapy and after chemotherapy. Results Before chemotherapy,RI was 〈 0. 5 in the two groups. RI increased significantly after chemotherapy in the endometial carcinoma group,and much higher than that of the ovarian cancer group（ P 〈 0. 05）. Before chemotherapy,CA125 levels of the two groups were very high,but with no statistical significance（ P〉0. 05）,but after chemotherapy,CA125 level in endometrial cancer group declined more dramatically than that of the ovarian cancer group（ P 〈 0. 05）,with statistical significance; In the endometrial carcinoma group, with the increase of RI, CA125 decreased. Blood flow RI and CA125 levels in tumor area had a good correlation（ r = 0. 75,P = 0. 001）,but the correlation between the two indexes was not seen obviously in the ovarian cancer group（ P〉0. 05）. Conclusion There are significant differences in the blood flow spectrum of uterine and adnexal malignant tumor before and after chemotherapy,and uterine artery resistance index and CA125 are helpful to judge the curative effect of chemotherapy in endometrial cancer patients,which are of certain clinical value.
