ICU患者临终生存质量及其影响因素研究

Study on the Quality of Life and Influencing Factors of ICU Patients at End Stage of Life

目的调查ICU患者临终阶段生存质量的现状并分析其影响因素。方法采用便利抽样法抽取温州市两所三级甲等医院ICU的住院患者176名,采用一般情况调查表、Mc Gill生存质量量表(Mc Gill)和姑息护理知识量表(PCQN)进行调查,使用多元回归分析ICU患者临终阶段生存质量的影响因素。结果 ICU患者临终阶段生存质量总分为21.45±7.23分,多元回归分析显示,文化程度、疾病了解程度、医疗负担和ICU医务人员的姑息护理知识是ICU患者临终阶段生存质量的主要影响因素,共解释47.8%的变异量。结论临床工作者应关注ICU患者的临终阶段生存质量,尤其是文化程度低、医疗负担重及对疾病了解程度高的患者的临终生存质量,提高ICU医务人员的姑息护理知识水平,以改善ICU患者的临终阶段生存质量。

Objective To investigate the status quo and influencing factors of quality of life in intensive care unit （ICU） patients who were at end stage of life. Methods A total of 176 inpatients at end stage of life were included using convenience sampling from ICU of two first class hospitals in Wenzhou. They were asked to report their demographic factors, quality of life （using McGill scale） and knowledge about palliative nursing （using PCQN scale）. Then we used multivariable regression model to investigate influencing factors of quality of life in ICU patients at end stage of life. Results The overall score of quality of life for ICU patients at end stage of life was 21.45 ＋ 7.23. Multiple linear regression analysis showed that education level, knowledge level about disease, medical burden and knowledge level about palliative nursing in ICU medical staff were the main influencing factors of quality of life in ICU patients at end stage of life （R square=47.8%）. Conclusion Clinicians should pay more attention to life quality of ICU patients at end stage of life, especially those with lower education level, higher medical burden and those who knew the disease better. In addition, improving the knowledge level of palliative nursing of ICU medical staff helps to perfect the life quality of ICU patients at end stage of life.
