QT离散度、心率变异性、脉搏波传导速度在心力衰竭患者中的应用价值

Application value of QT dispersion,heart rate variability and pulse wave velocity in patients with heart failure

目的探讨QT离散度(QTd)、心率变异性、脉搏波传导速度检查在心力衰竭患者中的临床应用价值。方法选择2015年1~12月期间我院收治的65例心力衰竭患者作为观察组,另选择同期在医院进行体检的62例健康者作为对照组,分别对两组受检者的心电生理及脉搏波传导速度进行检查和比较分析。结果与对照组比较,观察组患者QTd(92.37±10.51)ms和JT离散度(JTd)(90.68±10.17)ms均明显提高,差异均具有统计学意义(P<0.05);观察组患者SDNN(72.29±9.43)ms、SDANN(63.87±8.95)ms、r MSSD(20.64±4.90)ms均明显降低,差异均具有统计学意义(P<0.05);观察组心力衰竭患者LF(322.65±57.18)ms2/HZ和LF/HF(1.89±0.73)均明显提高,而HF(170.83±24.52)ms2/HZ则明显降低,差异均具有统计学意义(P<0.05);观察组患者ba-PWV(1 795.40±187.32)cm/s明显提高,差异具有统计学意义(P<0.05)。结论 QT离散度、心率变异性、脉搏波传导速度检查对于心力衰竭患者的病情诊断和疗效评价均具有十分重要的临床意义。

Objective To probe into the application value of QT dispersion（QTd）, heart rate variability andpulse wave velocity in patients with heart failure. Methods From January 2015 to December 2015, 65 patients with heartfailure were selected as the study group, and 62 healthy subjects were selected as the control group. The electrophysiologyand pulse wave velocity of the patients in study group and the control group were examined and compared. Results QTdand JT dispersion（JTd） were（92.37±10.51） ms and（90.68±10.17） ms in the study group, significantly higher than those inthe control group（P〈0.05）. Standard deviation of NN intervals（SDNN）, standard deviation of the averages of NN intervals（SDANN）, root mean squared successive differences（r MSSD） were（72.29±9.43） ms,（63.87±8.95） ms,（20.64±4.90） ms inthe study group, which were significantly decreased compared with those in the control group（P〈0.05）. LF, LF/HF were（322.65±57.18） ms2/HZ and was（1.89±0.73） in the study group, which were increased significantly compared with those inthe control group, while HF were（170.83±24.52） ms2/HZ in the study group, which were decreased significantly（P〈0.05）.ba-PWV in the study group were（1 795.40±187.32） cm/s, significantly higher than that in the control group（P〈0.05）. Conclusion QT dispersion, heart rate variability and pulse wave velocity have a very important clinical significance in dis-ease diagnosis and efficacy evaluation for patients with heart failure.
