子宫肌瘤剔除术后复发的影响因素分析

Analysis of risk factors of recurrence after myomectomy

目的探讨子宫肌瘤剔除术后复发的影响因素。方法回顾性分析2012年5月至2014年12月期间渭南市妇幼保健院妇产科收治的324例子宫肌瘤患者的临床资料,所有患者均接受子宫肌瘤剔除术治疗,根据术后是否复发将其分为复发组(n=79)与未复发组(n=245),分析术后复发的影响因素。结果本组患者子宫肌瘤剔除术后复发率为24.38%;单因素分析显示,年龄(手术时)、体重指数(BMI)、肌瘤个数、肌瘤大小等因素与子宫肌瘤剔除术后复发相关,差异均有统计学意义(P<0.05);经多元逐步回归分析显示,年龄、肌瘤个数、肌瘤大小是影响子宫肌瘤剔除术后复发的独立危险因素(P<0.05)。结论子宫肌瘤剔除术后复发率较高,年纪轻、肌瘤多发、肌瘤大是导致子宫肌瘤剔除术后复发的影响因素。

Objective To explore the risk factors of recurrence after myomectomy. Methods A total of 324 patients of uterine fibroids from May 2012 to December 2014 in Department of Obstetrics and Gynecology in our hospital were analyzed retrospectively. According to whether with postoperative recurrence, the patients were divided into recurrence group（n=79） and non-recurrence group（n=245）. Related factors of postoperative recurrence were analyzed by single factor analysis. Results The recurrence rate of myomectomy was 24.38%. Single factor analysis showed that age（at surgery）, body mass index（BMI）, size and number of uterine fibroids were related with recurrence after myomectomy（P〈0.05）.Multiple stepwise regression analysis showed that age, number of uterine fibroids, size of fibroids were the independent risk factors for recurrence after myomectomy（P〈0.05）. Conclusion The recurrence rate after myomectomy is generally high.The factors related to recurrence after myomectomy are younger age, multiple fibroids, and large fibroids.
