目的探索推进医疗机构联合体模式的推广应用,并提出发展方向。方法总结医疗机构联合体模式的做法和推进基层慢性病防控的成效。结果医联体模式对基层慢性病防控的成效,主要表现在提高了基层医疗卫生机构的居民就诊意愿,提升了基层医疗卫生服务机构的慢性病防控能力,推进了区域内各级医疗卫生机构慢性病医防整合,为基层慢性病防控做出了有益的探索。结论医联体模式对推进基层慢性病防控作用凸显,需要在今后的工作中不断完善和推广。

Objective To promote the application of regional medical institutions association model and propose future development direction. Methods The practice of regional medical institutions association model and the achievements on chronic disease prevention and control were summarized in the paper. Results In recent years,regional medical institutions association model had made initial achievements on chronic disease prevention and control which characterized by increasing the utilization wishes of community health services,improving the capacity of prevention and control of chronic diseases at primary medical and health service institutions,and promoting the Clinic-Prevention-Integration. Conclusion Regional medical institutions association model has significant promotion on chronic disease prevention and control in community health services,so it needs further improvement and extension.
