摘要
目的研究联合检测血红素加氧酶1(heme oxygenase 1,HO-1)、鳞状细胞癌抗原(Squamous cell carcinoma antigen,SCC)对食管癌的诊断价值及对放化疗疗效的反应。方法选取鹤壁市人民医院2015年2月—2016年2月收治的100例行放化疗治疗的食管癌患者作为研究组,另随机选取同期体检的26例健康者作为对照组。两组人员均在入院时及化疗后采集静脉血并用酶联免疫吸附法进行血清HO-1、SCC检测。观察不同临床分期、病理分级食管癌患者血清HO-1、SCC表达,比较放化疗前后血清HO-1、SCC浓度变化。计量资料比较采用单因素方差分析,采用ROC曲线分析肿瘤指标进行诊断准确性判断,P<0.05为差异有统计学意义。结果研究组血清HO-1、SCC表达随临床分期升高而增加,差异有统计学意义(均P<0.05);研究组血清HO-1随分化程度降低而减少,血清SCC表达随分化程度降低而增加,差异有统计学意义(均P<0.05);ROC曲线:血清HO-1、SCC在食管癌诊断方面具有一定应用价值,联合检测敏感性、特异性与药时曲线下面积(area under the curve,AUC)均高于单独检测;研究组放化疗后血清HO-1、血清SCC表达均低于放化疗前,差异有统计学意义(均P<0.05);研究组放化疗前后血清HO-1、SCC表达均高于对照组,差异有统计学意义(均P<0.05)。结论食管癌患者血清HO-1、SCC在不同临床分期、病理分级之间表达存在一定差异,在一定情况下降低HO-1、SCC表达能阻止肿瘤细胞的增殖与转移。
Objective To study the diagnostic value of joint detection of HO-1,SCC in esophageal cancer and chemoradiotherapy(CRT) efficacy.Methods A total of 100 patients with esophageal cancer took CRT in Hebi People's Hospital from February 2015 to February 2016 were selected as research group,and 26 healthy controls were randomly selected as control group.Serum HO-1 and SCC were detected by enzyme linked immunosorbent assay.The serum HO-1,SCC expression of different clinical stages and pathological grading in esophageal cancer patients were observed.The changes of HO-1,SCC concentration before and after CRT were compared.Measurement data were compared by t test.ROC curve was used to analyze tumor indicators to determine the accuracy of diagnosis.P〈0.05 was considered statistically significant.Results Expressions of serum HO-1 and SCC in research group increased with the increase of clinical stage(all P〈0.05).The serum HO-1 of research group decreased with the decrease of differentiation degree,and serum SCC increased with the decrease of differentiation degree(all P〈0.05).The sensitivity,specificity and area under the curve of combined detection were higher than those of single detection.The levels of serum HO-1 and serum SCC in the research group were lower than those before CRT(all P〈0.05).The serum levels of HO-1 and SCC in the research group were higher than those in the control group before and after CRT(all P〈0.05).Conclusion There were some differences in the expression of serum HO-1 and SCC between different clinical stages and pathological grades in patients with esophageal cancer.Under certain circumstances,reducing the expression of HO-1 and SCC can prevent the proliferation and metastasis of tumor cells.
