桂林地区不同季节对新生儿高促甲状腺素血症筛查的影响 被引量:1

The Effect of different seasons on screening of neonatal high thyrotropinemia in Guilin area

摘要 目的调查不同季节出生的新生儿促甲状腺激素(thyroid stimulating hormone,TSH)浓度,探讨不同季节与新生儿高促甲状腺素血症(high thyrotropinemia,HT)发病率的关系。方法 2015年3月—2017年2月采集128 962名新生儿足跟血,制成滤纸血片检测促甲状腺素浓度,进行先天性甲状腺功能减退症(congenital hypothyroidism,CH)筛查的同时,依据新生儿促甲状腺激素结果,筛查高促甲状腺素血症,统计不同季节出生的新生儿高促甲状腺素血症假阳性率和发病率,并进行比较。多个样本间率的比较采用χ~2检验,P<0.05为差异有统计学意义。结果春、夏、秋、冬季高促甲状腺素血症假阳性率分别为0.91%、0.61%、1.10%、2.07%,各季节间高促甲状腺素血症假阳性率比较,差异有统计学意义(P<0.05);春、夏、秋、冬季高促甲状腺素血症发病率分别0.11‰、0.19‰、0.22‰、0.24‰,各季节高促甲状腺素血症发病率比较,差异无统计学意义(P>0.05)。结论不同季节对高促甲状腺素血症发病率无影响。 Objective To investigate the levels of thyroid stimulating hormone(TSH)of neonates who were born in different quarters,and to explore the relationship between different quarter and the incidence of neonates high thyrotropinemia(HT).Methods The heel blood of 128 962 newborns were collected from March 2015 to February 2017,and dried blood spots on filter paper were made,then the thyroid stimulating hormone levels in dried blood spots were determined for congenital hypothyroidism.At the same time,high thyrotropinemia,high thyrotropinemia false positive rate and incidence of neonates who were born in different quarters were analyzed according to the results of newborns thyroid stimulating hormone.Enumeration data were compared by χ^2 test.P〈0.05 was considered statistically significant.Results The false positive rates of high thyrotropinemia in spring,summer,autumn and winter were 0.91%,0.61%,1.10%,2.07%.There was significant difference in false positive rate of high thyrotropinemia among different quarters(P〈0.05).The incidence of high thyrotropinemia in spring,summer,autumn and winter were 0.11‰,0.19‰,0.22‰,0.24‰.There was no statistically significant difference in the incidence of high thyrotropinemia among quarters(P〉0.05).Conclusion There was no effect on the incidence of high thyrotropinemia among different quarters.
