冠心病患者尿微量白蛋白/肌酐比值、胱抑素C与SYNTAX评分之间的相关性 被引量:4

摘要 目的分析冠状动脉粥样硬化性心脏病(冠心病)患者快速尿微量白蛋白/肌酐比值(UACR)、胱抑素C(Cys-C)与冠状动脉病变SYNTAX评分间的相关性。方法回顾性分析解放军总医院2016年10月~2017年3月收治的184例冠心病患者资料,男性122例,女性62例,年龄39~84岁,平均(56.5±10.8)岁。患者入院后行冠状动脉造影检查,参照SYNTAX冠状动脉评分系统进行评分。按照SYNTAX评分结果进行分组,低评分组(0~22分,140例)、中评分组(23~32分,34例),高评分组(≥33分,10例)。患者入院后留取晨尿检测UACR,抽取清晨空腹静脉血检测Cys-C水平。结果中评分组和高评分组UACR水平明显高于低评分组,高评分组Cys-C水平明显高于低评分组,差异有统计学意义(P均<0.05)。SYNTAX评分与Cys-C、UACR呈正相关,相关系数分别为0.12、0.31(P<0.05)。通过多元线性回归分析,Cys-C(β=0.212,P=0.031)、UACR(β=0.212,P<0.001)是SYNTAX评分的影响因素。结论冠心病患者尿微量白蛋白/肌酐比值、胱抑素C与SYNTAX评分之间存在正相关关系,均为SYNTAX评分的影响因素。 Objective To analyze the correlation among urea albumin creatinine ratio (UACR), cystatin C (Cys C) and SYNTAX scores of coronary artery lesions in patients with coronary heart disease (CHD). Methods The data was retrospectively analyzed from CHD patients (n=184, male 122, female 62, aged from 39 to 84 and average age=56.5±10.8) in the Chinese PLA General Hospital from Oct. 2016 to Mar. 2017. All patients were given examination of coronary angiography (CAG) after hospitalization, and were scored based on SYNTAX scoring system. The patients were divided, according to SYNTAX scores, into low-score group (0-22, n=140), mid-score group (23-32, n=34) and high-score group (≥33, n=10). The level of UACR in morning urine and Cys-C level in morning fasting venous blood were detected in patients after hospitalization. Results The level of UACR was significantly higher in mid-score group and high-score group than that in low-score group, and Cys-C level was significantly higher in high-score group than that in low-score group (all P〈0.05). SYNTAX scores was positively correlated to Cys-C (r=0.12) and UACR (r=0.31, P〈0.05). The analysis of multiple linear regression showed that Cys-C (β=0.212, P=0.031) and UACR (β=0.212, P〈0.001) were influence factors on SYNTAX scores. Conclusion There is a positive correlation among UACR, Cys C and SYNTAX scores in CHD patients, and UACR and Cys C are influence factors on SYNTAX scores. [
