嗜酸性粒细胞与冠状动脉慢血流的关系 被引量:3

摘要 目的探讨嗜酸性粒细胞计数与冠状动脉慢血流(SCF)之间的关系。方法回顾性分析北京市门头沟区医院心内科于2013年1月~2016年12月间因胸痛接受冠状动脉造影(CAG)检查的494例患者资料,男性163例,女性331例。CAG正常或接近正常的SCF患者62例纳入病例组,冠状动脉血流正常的432例患者纳入对照组。患者基线资料、血液生化检查和血常规检查结果均从住院数据库中获得。结果SCF组嗜酸性粒细胞计数明显高于对照组[(0.19±0.05)×109/L vs.(0.11±0.04)×109/L],差异有统计学意义(P<0.05)。单因素回归分析显示嗜酸性粒细胞计数(OR=5.376,95%CI:3.660~7.897,P<0.001)是SCF的危险因素。多因素回归分析显示:嗜酸性粒细胞计数(OR=7.567,95%CI:4.634~12.350,P<0.001)和空腹血糖(OR=1.266,95%CI:1.087~1.474,P=0.002)是SCF的危险因素。SCF患者的嗜酸性粒细胞计数与冠状动脉血流速度呈正相关,相关系数r=0.370(P<0.001)。嗜酸性粒细胞计数诊断SCF的敏感度为96.8%,特异度为64.8%,受试者工作特征曲线下面积(AUC)为0.879(95%CI:0.843~0.916)。结论嗜酸性粒细胞计数和冠状动脉慢血流相关,嗜酸性粒细胞升高对冠状动脉慢血流可能有一定的预测价值。 Objective To discuss the relationship between count of eosinophil and slow coronary flow (SCF). Methods The medical data from patients (n=494, male 163 and female 331) received coronary angiography (CAG) due to chest pain were retrospectively analyzed in the Department of Cardiology of Hospital of Beijing Mentougou District from Jan. 2013 to Dec. 2016. SCF patients with normal or almost normal CAG were chosen into case group (n=62) and those with normal coronary flow were chosen into control group (n=432). The baseline data, and results of blood biochemical examinations and blood routine examinations were all obtained from inpatient database. Results The count of eosinophil was significantly higher in SCF group than that in control group [(0.19±0.05)×109/L vs. (0.11±0.04)×109/L, P〈0.05]. The results of single-factor regression analysis showed that count of eosinophil was the risk factor of SCF (OR=5.376, 95%CI: 3.660~7.897, P〈0.001). The results of multi-factor regression analysis showed that count of eosinophil (OR=7.567, 95%CI: 4.634~12.350, P〈0.001) and fasting plasma glucose (OR=1.266, 95%CI: 1.087~1.474, P=0.002) were the risk factor of SCF. The count of eosinophil was positively correlated to coronary flow velocity (r=0.370, P〈0.001) in SCF patients. The sensitivity of count of eosinophil was 96.8%, specificity was 64.8% and AUC of ROC was 0.879 (95%CI: 0.843~0.916) in diagnosis of SCF. Conclusion The count of eosinophil is related to SCF, and increase of eosinophil may be valuable to the prediction of SCF.
