摘要 目的探究肾上腺髓质素前体中段肽(MR-pro ADM)、正五聚蛋白-3(PTX-3)、脑钠肽(BNP)、ST-2水平在心力衰竭患者中的变化及意义。方法选择2015年9月~2017年6月于陕西省铜川市人民医院进行治疗的心力衰竭患者90例,男性52例,女性38例。依据纽约心脏协会(NYHA)心功能分级分为:NYHAⅡ组(28例),NYHAⅢ组(32例)以及NYHAⅣ组(30例)。同期选择本院体检健康者40例纳入对照组。对照组入组后,所有患者治疗前以及治疗后采用酶联免疫吸附法测定PTX-3、ST-2、MR-pro ADM水平,BNP水平检测采用电化学发光免疫法。结果对照组、NYHAⅡ组、NYHAⅢ组以及NYHAⅣ组MR-pro ADM、PTX-3、BNP、ST-2水平均呈升高趋势,各组间比较差异有统计学意义(P均<0.05)。与治疗前比较,治疗后各组患者血清MR-pro ADM、PTX-3、BNP、ST-2水平均显著降低,差异具有统计学意义(P均<0.05)。结论心力衰竭患者血清MR-pro ADM、PTX-3、BNP、ST-2水平均显著升高,且随着患者心功能下降其水平越高;检测血清MR-pro ADM、PTX-3、BNP、ST-2水平有可能作为判断心力衰竭病情的手段之一。 Objective To study the changes and significance of levels of serum MR-pro-adrenomedullin (MR-proADM), pentraxins-3 (PTX-3), brain natriuretic peptide (BNP) and ST-2 in patients with heart failure (HF). Methods HF patients (n=90, male 52 and female 38) were chosen from the People’s Hospital of Tongchuan City of Shaanxi Province from Sept. 2015 to June 2017. The patients were divided, according to NYHA grading, into NYHA Ⅱ group (n=28), NYHA Ⅲ group (n=32) and NYHA Ⅳ group (n=30), and health controls (n=40) were chosen into control group at the same time. The levels of PTX-3, ST-2 and MR-proADM were detected by using enzyme-linked immunosorbent assay (ELISA), and BNP level was detected by using electro-chemiluminescence immunoassay (ECLIA) in all groups before and after treatment. Results The levels of MR-proADM, PTX-3, BNP and ST-2 all showed an ascending trend in all groups, and the comparison among all groups had statistical significance (all P〈0.05). Compared with before treatment, levels of MR-proADM, PTX-3, BNP and ST-2 all decreased significantly in all groups after treatment (all P〈0.05). Conclusion The levels of MR-proADM, PTX-3, BNP and ST-2 all increased significantly in HF patients, and will be higher along with the decrease of heart function. The detection of levels of MR-proADM, PTX-3, BNP and ST-2 can be one of methods for diagnosing HF severity.
