目的:介绍压力性损伤链式管理的临床实践并分析其成效。方法:对患者的压力性损伤风险控制从就诊开始直至出院后,其中包括了急诊科、ICU、手术室、病区及延续护理的每个过程。结果:压力性损伤链式管理有利于在全院范围内对皮肤护理和压力性损伤监控进行科学管理。住院患者压力性损伤的发生率逐年下降,低于全国及江苏省平均水平。结论:通过压力性损伤链式管理的临床运用,建立了组织管理链和环节管理链,加强了压力性损伤护理的横向管理和控制,形成了无缝隙、连续的链式管理结构。

Objective： To explore the application mode of pressure injury chain management in clinical practice. Methods： Control the risk of pressure injury on patients from the treatment to the discharge, including the emergency department, the ICU, the operating room, the ward and the continuation of nursing. Results： It is conducive to the scientific management of skin care and pressure injury monitoring in the whole hospital. The incidence of pressure injury in hospitalized patients decreased year by year, which was lower than the average level of the national and Jiangsu province. Conclusion： Through the chain management of pressure injury in clinical use, the organization management chain and link management chain are established, the lateral management and control of the pressure injury care are strengthened, and the continuous chain management structure is formed.
