CA199、CA125联合检测在卵巢肿块鉴别及病情判断中的价值

Value of combined detection of CA199 and CA125 in identification and disease condition judgment of ovarian masses

目的探讨糖类抗原199(CA199)、糖类抗原125(CA125)联合检测在卵巢肿块鉴别及病情判断中的价值,为后续临床治疗提供参考依据。方法收集2013年10月-2017年1月在该院就诊发现卵巢肿块的患者168例为研究对象,分为卵巢癌组(120例),卵巢成熟畸胎瘤组(48例),另取选取同期在该院进行体检的健康志愿者50例为对照组。对比3组研究对象血清中肿瘤标志物CA199、CA125含量,卵巢超声血流动力学参数水平,血清血管新生指标含量等差异。采用Pearson检验评估卵巢癌患者血清CA199、CA125含量与卵巢超声血流动力学参数水平,血清血管新生指标含量的相关关系。结果卵巢癌组血清中CA199、CA125的含量显著高于卵巢成熟畸胎瘤组,对照组(P<0.05);卵巢癌组搏动指数(PI)、阻力指数(RI)低于卵巢成熟畸胎瘤组,对照组(P<0.05);卵巢癌组血清中血管内皮生长因子(VEGF)、缺氧诱导因子-1α(HIF-1α)、基质金属蛋白酶-2(MMP-2)的含量高于卵巢成熟畸胎瘤组,对照组(P<0.05)。Pearson检验发现,卵巢癌患者血清中CA199、CA125的含量与卵巢超声血流动力学参数水平,血清血管新生指标含量均直接相关。结论卵巢良恶性肿块患者血清CA199、CA125含量差异存在统计学意义(P<0.05),且其含量与肿瘤恶性程度密切相关,可作为卵巢癌诊断及病情判断严重程度的重要指标。

Objective To explore the value of combined detection of CA199 and CA125 in identification and disease condition judgment of ovarian masses,provide a reference basis for subsequent clinical treatment. Methods A total of 168 patients with ovarian masses treated in the hospital from October 2013 to January 2017 were collected and divided into ovarian cancer group（ 120 patients） and ovarian mature teratoma group（ 48 patients）,50 healthy volunteers receiving physical examination in the hospital during the same period were selected as control group. The levels of CA199,CA125,ovarian hemodynamics parameters,serum angiogenesis indexes in the three groups were compared. Pearson test was used to evaluate the correlations between the levels of serum CA199,CA125 and ovarian hemodynamics parameters,serum angiogenesis indexes in ovarian cancer group. Results The levels of serum CA199 and CA125 in ovarian cancer group were statistically significantly higher than those in mature ovarian teratoma group and control group（ P 0. 05）. Pulsatility index（ PI） and resistance index（ RI） in ovarian cancer group were statistically significantly lower than those in mature ovarian teratoma group and control group（ P0. 05）. The levels of serum vascular endothelial growth factor（ VEGF）,hypoxia-inducible factor-1α（ HIF-1α）,matrix metalloproteinase-2（ MMP-2） in ovarian cancer group were statistically significantly higher than those in mature ovarian teratoma group and control group（ P0. 05）. Pearson test showed that serum levels of CA199 and CA125 in ovarian cancer group were directly related to the levels of ovarian hemodynamics parameters,serum angiogenesis indexes. Conclusion There are statistically significant differences in the levels of serum CA199 and CA125 between patients with benign ovarian tumors and patients with malignant ovarian tumors,the levels of serum CA199 and CA125 are closely correlated with the degree of malignancy,which can be used as important indicators for diagnosis and disease condition judgment of ovarian cancer.
