针药结合综合方案治疗社区卒中后抑郁的疗效观察

Curative Effect of Acupuncture Combined with Medicine in Treatment of PSD

目的:观察针药结合综合方案治疗社区卒中后抑郁的疗效。方法:将60例社区PSD患者随机分为两组:治疗组和对照组,每组30例。治疗组采用针药结合的综合治疗方案进行辨证分型,选择合适的方剂和合适的穴位进行中药和针灸治疗,中药汤剂每日1剂,针灸每周3次;对照组给予黛力新治疗,每天2片。治疗周期均为8周。治疗前和治疗第1、2、4、8周分别进行HMAD、SDS、Barthel指数、SSQOL评分,从而对患者的卒中情况和抑郁状况进行评价。结果:两组(治疗组和对照组)治疗后2,4,8周与治疗前相比,Barthel指数、SSQOL评分均显著提高(P<0.05),组间则没有差异性(P>0.05);两组治疗后2周与治疗前相比,HAMD评分、SDS评分均降低(P<0.05);两组治疗后4,8周与治疗前相比,HAMD评分、SDS评分均显著降低(P<0.01);组间比较没有差异性(P>0.05)。治疗组与对照组比较,有效率没有差异(P>0.05)。结论:针药结合的综合治疗方案对社区PSD有显著的改善作用。

Objective ： To observe the curative effect of the treatment about the PSD by a comprehensive scheme combined with acupuncture and medicine. Methods：We divided 60 PSD patients into two groups randomly,30 cases in each group. The treatment group was treated by the comprehensive scheme and syndrome differentiation to select the appropriate formula and the appropriate points for acupuncture, a daily dose of the decoction of Chinese medicine and 3 times a week of acupuncture and moxibustion. The control group received the treatment of Deanxit for 2 tablets a day. The treatment period was 8 weeks in a row. To score the 24 i- tem HMAD,SDS,BI,SSQOL in the lst,2nd,4th and 8th week respectively before and after treatment. Results：The Barthel index and SSQOL score were increased obviously both in treatment group and control group in 2nd,4th and 8th week after treatment compared with those before treatment（P 〈 0. 05）but the two groups had no difference（P 〉 0.05）. HAMD and SDS scores were decreased obviously both in treatment group and control group in 2nd week after treatment compared with those before treatment. And HAMD and SDS scores in two groups were obviously decreased 4th and 8th week after treatment（ P 〈 0. 01 ）but the two groups had no difference（P 〉0. 05）. The effective rates of the two groups had no difference（P 〉0. 05）. Conclusion：The compre- hensive therapy of acupuncture combined with medicine was effective on PSD.
