2013年云南省肿瘤登记地区恶性肿瘤发病与死亡分析 被引量:20

摘要 [目的]描述云南省2013年肿瘤登记地区恶性肿瘤发病与死亡情况,为云南省癌症防治工作提供基础信息。[方法]按照肿瘤登记质量控制标准,对云南省11个肿瘤登记处报告的2013年数据进行系统评估。对符合质量要求的5个登记处数据进行汇总统计,分城乡、性别、年龄及癌症部位分别计算发病率和死亡率。[结果]2013年纳入分析的5个登记处覆盖人口共2 141 576人,占全省2013年人口数的4.52%。病理诊断比例(MV%)为64.51%,只有死亡医学证明书比例(DCO%)为1.49%,死亡发病比(M/I)为0.61。全省肿瘤登记地区恶性肿瘤发病率为213.16/10万(城市241.45/10万,农村190.92/10万),中标率为156.30/10万,世标率为151.05/10万,累积率(0~74岁)为16.97%。全省肿瘤登记地区恶性肿瘤死亡率为131.07/10万(城市148.52/10万,农村117.35/10万),中标死亡率为91.21/10万,世标死亡率为89.57/10万,累积率(0~74岁)为10.11%。肺癌、女性乳腺癌、结直肠癌、宫颈癌、肝癌、胃癌、子宫体癌、前列腺癌、脑瘤和卵巢癌是云南省主要恶性肿瘤,约占全部新发病例的67.86%。肺癌、肝癌、结直肠癌、胃癌、女性乳腺癌、宫颈癌、胰腺癌、白血病、脑瘤及胆囊癌是云南省主要恶性肿瘤死因,约占全部肿瘤死亡病例的78.55%。[结论]应加强对肺癌、肝癌、结直肠癌、胃癌、宫颈癌、女性乳腺癌等主要恶性肿瘤的防控。 [Purpose] To analyze the cancer incidence and mortality in Yunnan cancer registration areas in 2013.[Methods] Data submitted from 11 registries were evaluated according to cancer registration quality control standards.The qualified data of 5 registries was stratified by areas,gender,age groups and cancer sites,and the incidence and mortality were calculated.[Results]The 5 cancer registration areas covered a population of 2 141 576,accounting for 3.21% of whole population of Yunnan province in 2013.The morphology-verified cancer cases(MV%) accounted for 64.51%,while 1.49% of the incident cases were identified only through death certification records(DCO%).The crude incidence of cancer was 213.16/105(urban 241.45/105,rural 190.92/105),the age-standardized incidence rates of cancer by Chinese standard population and by world standard population were 156.30/105 and 151.05/105,with the cumulative incidence rate(0 ~74 years) of 16.97%.The crude mortality was 131.07/105(urban 148.52/105,rural 117.35/105),the age-standardized mortality rates of cancer by Chinese standard population and by world standard population were 91.21/105 and 89.57/105,with the cumulative mortality rate(0 ~ 74 years) of 10.11%.The mortality to incidence ratio was 0.61.The most common sites of cancers were lung,female breast,colon and rectum,cervix,liver,stomach,uterus,prostate,brain and ovary,which accounted for 67.86% of all the cancer cases.Lung cancer,liver cancer,colorectal cancer,stomach cancer,female breast cancer,cervix cancer,pancreas cancer,leukemia,brain tumor and gallbladder cancer were the leading causes of cancer death,accounting for 78.55% of all cancer deaths.[Conclusion] Prevention and control of the most common cancers such as lung cancer,liver cancer,colorectal cancer,stomach cancer,cervix cancer and female breast cancer should be strengthened in Yunnan province.
