不同治疗周期骶管滴注联合四步松解手法治疗腰椎间盘突出症的临床效果分析

Clinical Effect Analysis on Instillation of Sacral Canal with Four Step Releasing Manipulation on Prolapse of Lumbar Intervertebral Disc in Different Treatment Cycles

目的观察不同治疗周期骶管滴注联合四步松解手法治疗腰椎间盘突出症患者的临床效果。方法按照随机数字表法将患者随机分为对照组和观察组各144例,对照组患者采用骶管滴注进行治疗,观察组患者采用骶管滴注联合四步松解手法进行治疗,在治疗前、治疗1周、2周、12周后分别记录患者的疼痛评分(VAS)、JOA腰椎疾患治疗成绩评分、Oswestry功能障碍指数问卷表(ODI)及患者对治疗方法的满意度。结果在治疗1、2、12周后,试验组患者总有效率分别为74.31%、86.81%、94.44%,均高于对照组的52.78%、72.22%、87.50%(P<0.05)。不同治疗周期VAS评分及ODI总评分在治疗后均显著下降,JOA评分在治疗后显著增加,治疗周期越长,变化幅度越大,且观察组的变化幅度均高于对照组(P<0.05)。观察组患者对治疗方法的满意度分别达63.89%、82.64%、93.06%,均高于对照组的48.61%、58.33%、79.17%(P<0.05)。结论骶管滴注联合四步松解手法治疗腰椎间盘突出症的疗效显著,安全可行。

Objective： To observe the clinical effect of instillation of sacral canal with four step releasing manipulation on prolapse of lumbar intervertebral disc in different treatment cycles. Methods： 288 cases were randomly divided into the control group and the experimental group according to the random number table method, 144 cases in each. The control group were treated with instillation of sacral canal. The experimental group were treated with instillation of sacral canal combined with the four-step releasing manipulation. Before treatment, 1 week,2 weeks and 12 weeks after treatment,VAS,JOA,ODI and patients＇ satisfaction with treatment were accessed. Results： The total effective rate was 52.78% ,72.22% and 87.50% in the control group after treated for 1,2 and 12 weeks respectively. The total effective rate was 74.31% ,86.81% and 94.44% in the experimental group. There was significant difference between the two groups （P〈 0.05）. The VAS score and the total score of ODI were significantly decreased after treatment,and the JOA score increased significantly after treatment. The longer the treatment period was, the wider the change range was, and the change range of the experimental group was wider than that of the control group （P〈0.05）. The satisfaction rate of the treatment group was 63.89%,82.64% and 93.06% respectively,and that of the control group was 48.61%,58.33% and 79.17% respectively after treated for 1,2 and 12 weeks. There was significant difference between the two groups （P〈 0.05）. Conclusion： Instillation of sacral canal with four step releasing manipulation on prolapse of lumbar intervertebral disc is effective and safe,worthy of clinical application.
