温经散寒散穴位贴敷治疗原发性痛经(寒凝血瘀证)的临床研究

Clinical Observation on Acupoint Application of Wenjing Sanhan Decoction on Primary Pysmenorrhea

目的观察温经散寒散穴位贴敷治疗原发性痛经(PD)的临床疗效。方法将100例PD(寒凝血瘀证)患者按随机数字表法分为治疗组和对照组各50例,治疗组给予温经散寒散于三阴交、神阙等穴位贴敷治疗,对照组给布洛芬咀嚼片口服治疗,治疗3个月经周期后评估疗效。结果治疗组治疗总有效率为91.67%,高于对照组的76.60%(P<0.05);治疗3个月经周期后,两组患者视觉模拟疼痛量表(VAS)和COX痛经症状评估量表(CMSS)评分均较治疗前改善明显(P<0.01);治疗组改善程度优于对照组(P<0.01);治疗3个月经周期后,治疗组患者前列腺素PGF2α和PGE2水平较治疗前明显改善(P<0.01);两组比较,治疗组与对照组差异无统计学意义(P>0.05)。结论温经散寒散穴位贴敷治疗可调节PD(寒凝血瘀证)患者血清前列腺素水平,改善痛经症状。

Objective： To observe the clinical effect of acupoint application of Wenjing Sanhan Decoction on primary dysmenorrhea. Methods： 100 patients with primary dysmenorrhea（type of blood stasis due to cold） were randomly divided into the treatment group and the control group （n = 50）. The treatment group received Wenjing Sanhan Decoction by acupoint application with Sanhuijiao （SP6）, Shenque （CVS） and other points. The control group was treated with Ibuprofen Chewables Tablets orally, and the curative effect was evaluated after 3 menstrual cycles. Results： The total effective rate was 91.67% in the treatment group and 76.60% in the control group,and the difference between the two groups was statistically significant （P〈 0.05）. After 3 menstrual cycles,the VAS and CMSS scores of the two groups were significantly improved compared with those before treatment （P〈 0.01 ）. Compared with the control group, the improvement degree of the treatment group was better than that of the control group in the two groups （P〈 0.01 ）. After treatment of 3 menstrual cycles,the levels of prostaglandin PGF2 alpha and PGE2 in treatment group were significantly improved compared with before treatment（P〈 0.01 ）;there was no significant difference between＇the two groups （P〉 0.05）. Conclusion： Wenjing Sanhan Decoction by acupoint application on primary dysmenorrhea （type of blood stasis due to cold） can adjust prostaglandin levels,and im- prove the symptom assessment of primary dysmenorrhea.
