基于中医辨证探析胸痛证治体系构建

急性胸痛快速、准确地鉴别诊断是急诊科诊疗的难点与重点,而胸痛中心的建立在胸痛早期明确诊断、危险评估、快速诊疗、降低死亡率等方面扮演重要角色,然胸痛中医辨证尚缺乏专文描述,为此本文通过胸痛临床症状以及基础病变情况,根据相关文献进行汇总分析,结果显示胸痛涵盖中医胸痹、猝心痛、肺胀、肺痈、悬饮、缠腰火丹等多个疾病范畴。中医急诊对于病情复杂,病因多样,病情变化急的胸痛具有系统的辨证思维、能明确虚实病机,阐明病位,指出扶正、祛邪以及扶正祛邪治则,指导胸痛临床辨证论治。因此,探析胸痛中医证治体系对现代胸痛中心建设、构建胸痛证治体系、早期评估病情、正确分诊、及时救治、降低死亡率和致残率具有重要意义。

The rapid and accurate diagnosis of chest pain is the most difficult and important point in the diagnosis and treatment of emergency department. At present the construction of the chest pain center plays an important role in the early diagnosis of emergency chest pain, risk assessment, rapid diagnosis and treatment, mortality reduction. However,the emergency chest pain ofsyndrome differentiation and treatment of Traditional Chinese Medicine （TCM） are insufficient. So ,this article through the clinical symptoms of chest pain and the basic lesion＇, according to the relevant literature summary analysis,the results show that emergency chest pain covers thoracie obstruction, sudden cardiac pain,lung swelling,lung earbunele, hanging drink ,wrapped around the fire and other areas of fire Darj. TCM emergency diagnosis of chest pain with eomplex disease, diverse causes and rapid changes of the disease has a systematic dialectical thinking. It can clarify the actual situation, clarify the disease position, and point out the principles of strengthening the body,removing pathogenic factors and strengthening the body and eliminating the evil and guiding the clinical dialectical treatment of chest pain. Therefore,it is of great signifieanee to explore the system of syndrome differentiation and treatment of chest pain as a modern center for the development of chest pain, construct a system of chest pain syndrome and assess the disease early, correct diagnosis, timely treatment, and reduce mortality and morbidity.
