护理干预在负压封闭引流治疗糖尿病足中的应用效果研究

The application effect of nursing intervention in the treatment of diabetic foot by vacuum sealing drainage

目的探讨护理干预在负压封闭引流治疗糖尿病足中的应用效果。方法选取2012年12月~2014年12月我院收治的104例糖尿病足患者作为研究对象,采用随机数字表法分为对照组和观察组,每组各52例。对照组实施常规护理,观察组实施护理干预。患者均行负压封闭引流治疗。比较两组患者的治疗依从性、恢复情况、住院情况、并发症情况以及护理满意度。结果观察组患者干预后血糖控制时间[(2.4±0.8)d]、创面愈合时间[(12.7±1.8)d]、敷料更换次数[(1.4±0.3)次]、住院时间[(14.7±2.5)d]、住院费用[(10327.5±1234.8)元]均明显少于对照组[(4.2±1.0)d、(17.6±2.3)d、(3.5±1.6)次、(19.8±3.1)d、(13 473.6±2490.3)元],敷料持续时间[(10.3±1.2)d]明显长于对照组[(5.4±0.7)d],并发症发生率(1.9%)明显低于对照组(15.4%),治疗依从性(98.1%)、护理满意度(96.2%)均明显高于对照组[(86.5%)、(82.7%)],差异均有统计学意义(P<0.05)。结论负压封闭引流治疗糖尿病足时,护理干预可缩短疗程,并提高安全性,能显著提高患者的治疗依从性和护理满意度,值得临床推广应用。

Objective To explore the effect of nursing intervention in the treatment of diabetic foot by vacuum sealing drainage.Methods A total of 104 patients with diabetic foot from December 2012 to December 2014 treated in our hospital were selected as the subjects.By a random number table,they were evenly divided into control group and obser- vation group,52 cases in each group.Patients in the control group were treated with routine nursing.And the patients in observation group were treated with nursing intervention.All of the patients were treated with vacuum sealing drainage. The treatment compliance,recovery,hospitalization,complications,and nursing satisfaction were compared between the two groups.Results In the observation group,the time of blood glucose control,wound healing time,dressing change,hos- pital stay,and hospital cost were （2.4±0.8） d,（12.7±1.8） d,（1.4±0.3） times,（14.7±2.5） d,（10 327.5±1234.8） yuan,which were significantly lower than those in the control group,（4.2±l.0） d,（17.6±2.3） d,（3.5±1.6） times,（19.8±3.1） d,and （13 473.6±2490.3） yuan acc0rdingly.The dressing duration in the observation group （[10.3±1.2]d） was shorter than that in the control group [（5.4±0.7）d].The incidence of complications,treatment compliance,and nursing satisfaction was 1.9%, 98.1%,and 96.2% respectively,all superior to those in the control group with 15.4%,86.5%,and 82.7%,the differences were statistically significant （P〈0.05）.Conclusion In the treatment of diabetic foot by vacuum sealing drainage,nursing intervention can shorten the course of treatment,improve safety,remarkably increase the treatment compliance and nurs- ing satisfaction,which is worthy of clinical application.
