双LVIS支架辅助栓塞治疗颅内宽颈动脉瘤临床研究 被引量:10

Therapeutic effects and safety of double LVIS stent-assisted coil embolization on intracranial wide-necked aneurysms

摘要 目的探讨双LVIS支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤的安全性和有效性。方法将54例颅内宽颈动脉瘤患者分为对照组(n=29)和观察组(n=25),对照组行单支LVIS支架辅助栓塞治疗,观察组行双LVIS支架辅助栓塞治疗,比较2组术后栓塞情况,随访6个月进行影像学检测,采用改良Rankin量表(mRS)评估患者神经功能缺损,以判断其预后。结果观察组动脉瘤Ⅰ级栓塞率显著高于对照组(P<0.05);6个月影像学结果显示观察组不显影率显著高于对照组,复发率低于对照组,差异均有统计学意义(P<0.05);mRS评分结果提示,观察组预后疗效优于对照组(P<0.05)。结论应用双LVIS支架辅助栓塞能够提高颅内宽颈动脉瘤致密栓塞率,降低复发率,安全有效。 Objective To evaluate the safety and efficacy of double LVIS stent-assisted coil embolization in treatment of intracranial wide-necked aneurysms. Methods Fifty-four patients with intracranial wide-neck aneurysm were divided into two groups: control group( n = 29) and observation group( n = 25). The patients in control group were treated by single LVIS stent-assisted embolization,however,the patients in control group were treated by double-LVIS stent-assisted embolization. The imaging examination was performed during 6-month follow up,and the neurologic impairment was assessed by improved Rankin scale( mRS) to evaluate patient's prognosis. Results The imaging results showed that the aneurysm gradeⅠembolism rate was 96. 0% in observation group,which was significantly higher than that( 72. 41%) in control group( P〈0. 05),moreover,the nonvisulization rate in observation group was significantly higher than that in control group,however,the relapse rate was significantly lower than that in control group( P〈0. 05). The mRS score results showed that patient's prognosis in observation group was superior to that in control group( P〈0. 05). Conclusion The double LVIS stent-assisted embolization in treatment of intracranial wide-necked aneurysms can improve the dense embolization rate,reduce relapse rate,with safety and effectiveness.
