克罗恩病患者低钙血症发生的危险因素分析 被引量:3

Risk factors for hypocalcemia in patients with Crohn's disease

摘要 目的探讨克罗恩病患者发生低钙血症的危险因素。方法 95例克罗恩病患者分为低钙血症(校正钙浓度<2.20mmol/L)组25例和正常血钙(校正钙浓度2.20~2.75mmol/L)组70例,比较2组年龄、性别比例、体质量指数(body mass index,BMI)、每日日照时间,血红蛋白、前蛋白、白蛋白、总蛋白、球蛋白、白/球比、血清谷丙转氨酶(glutamic pyruvic transaminase,GPT)、谷草转氨酶(glutamic oxaloacetic transaminase,GOT)、尿素、肌酐、血磷、血镁、血沉、三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、脂蛋白a水平,以及简化克罗恩病活动度(Crohns disease activity index,CDAI)评分、克罗恩病国内标准内镜(Chinese Drading System for Crohns Disease,CGSCD)评分,多因素logistic回归分析克罗恩病患者发生低钙血症的危险因素。结果低钙血症组每日日照时间≤2h比率(44.0%)、简化CDAI评分[(7.8±3.7)分]、CGSCD评分[(2.8±0.5)分]、血红蛋白[(128.4±30.0)g/L]、前蛋白[(218.7±57.4)g/L]、总蛋白[(74.2±5.5)g/L]、白蛋白[(45.2±2.8)g/L]、白/球比(1.6±0.3)、血镁[(0.9±0.1)mmol/L]、TC[(3.9±0.8)mmol/L]、HDL-C[(1.2±0.3)mmol/L]、LDL-C[(2.2±0.7)mmol/L]水平均高于正常血钙组[11.4%、(5.6±2.0)分、(2.3±0.8)分、(110.7±23.6)g/L、(151.9±56.9)g/L、(62.5±7.9)g/L、(34.2±5.1)g/L、1.2±0.2、(0.8±0.2)mmol/L、(3.2±0.8)mmol/L、(0.9±0.4)mmol/L、(1.6±0.5)mmol/L](P<0.05),BMI[(16.6±2.1)kg/m^2]、脂蛋白a[(133.9±23.8)mmol/L]低于正常血钙组[(19.3±2.4)kg/m^2、(297.6±84.5)mmol/L](P<0.05);2组年龄、性别比例、GOT、GPT、球蛋白、尿素、肌酐、血磷、血沉、TG比较差异无统计学意义(P>0.05);多因素logistic回归分析结果显示,每日日照时间≤2h(OR=6.86,95%CI:1.42~33.56,P=0.019)、简化CDAI评分≥7.0分(OR=3.10,95%CI:1.15~6.45,P=0.016)、CGSCD评分≥2.0分(OR=4.33,95%CI:1.28~14.62,P=0.018)是克罗恩病患者发生低钙血症的独立危险因素(P<0.05)。结论每日日照时间短,简化CDAI评分、CGSCD评分增高与克罗恩病患者发生低钙血症有关。 Objective To evaluate the risk factors for hypocaleemia in patients with Crohn's disease (CD). Methods Ninety-five patients with CD were divided into hypocalcemia group with correcting calciumS2.20 mmol/L (n= 25) and normal calcium group with correcting calcium 2.20 to 2.75 mmol/L (n= 70). The age, sex ratio, body mass index, daily sunshine exposure, hemoglobin, prealbumin, albumin, total protein, globulin, white/globule ratio, serum glutamic pyruvic transaminase (GPT), glutamic oxaloacetic transaminase (GOT), urea, creatinine, phosphorus, magnesium, erythrocyte sedimentation rate, triacylglycerol, total cholesterol, high-density lipoprotein cholesterol (HDL-C), low- density lipoprotein cholesterol (LDL-C), TC/HDL-C, total lipoprotein (a), Crohn's disease activity index (CDAI) score and Chinese Grading System for Crohn's Disease (CGSCD) score were compared between two groups. The risk factors for hypocalcemia were analyzed by multivariate logistic regression. Results The proportion of the sunshine exposure time 2 h/d (44.0%), CDAI score (7.8±3.7), CGSCD score (2.8±0.5), the hemoglobin level ((128.4±30.0) g/L), the prealbumin level ((218.7±57.4) g/L), the total protein level ((74.2±5.5) g/L), albumin level ((45.2±2.8) g/L),the white/globule ratio (1.6±0.3), magnesium concentration ((0.9±0.1) mmol/L), the total cholesterol level ((3.9±0.8) mmol/L), HDL-C level ((1.2±0.3) mmol/L) and LDL-C level ((2.2-t-0.7) mmol/L) in hypocalcemia group were significantly higher than those in normal calcium group (11.4%, 5.6±2.0, 2.3±0.8, (110. 7±23.6) g/L, (151.9± 56.9) g/L, (62.5±7.9) g/L, (34.2±5.1) g/L, 1.2±0.2, (0.8±0.2) mmol/L, (3.2±0.8) mmol/L, (0.9±0.4) mmol/L, (1.64-0.5) mmol/L) (P〈0.05), and body mass index ((16.6±2.1) kg/m^2 ) and total lipoprotein(a) level ((133.9±23.8) mmol/L) in hypocalcemia group were significantly lower than those in normal calcium group ((19.3± 2.4) kg/m2 , (297.6±-84.5) mmol/L) (P〈0.05). There were no significant difference in the age, sex ratio, GOT, GPT, globulin, urea, creatinine, phosphorus, erythrocyte sedimentation rate and TG between two groups (P〉O. 05). The multivariate logistic regression analysis showed that the insufficient sunshine exposure (sunshine exposure time 2 h/d) (OR=6.86, 95% CI: 1.42-33.56, P=0.019), CDAIscore 7.0 (OR=3.10, 95YOCI, 1.15-6.45, P=0.016), and CGSCD seore2. 0 (OR = 4. 33, 95% CI: 1. 28-14. 62, P= 0.018) were the independent risk factors for hypocalcemia in CD patients. Conchlsion The insufficient sunshine exposure, increased CDAI score and increased CGSCD score are the risk factors for hypocalcemia in CD patients.
