摘要
目的:研究并探讨磁共振成像(Magnetic resonance imaging,MRI)在诊断脑血管周围间隙扩大中的应用价值。方法:选取我院在2015年1月~2016年12月期间100例可能存在血管周围间隙扩大的2型糖尿病患者作为研究对象,所有患者均接受MRI检查,常规行T1WI、T2WI及T2WI-FLAIR序列扫描,部分再注入对比剂行增强扫描,对其MRI图像进行观察,分析脑血管周围间隙扩大的MRI表现,作出诊断,分析MRI对脑血管周围间隙扩大的诊断准确性。结果:100例患者中,共有43例患者的脑血管周围间隙直径超过3mm,被确诊为脑血管周围间隙扩大,MRI对脑血管周围间隙扩大的诊断准确性为100%。43例确诊患者的脑血管周围间隙均位于脑半球,以基底节区及放射冠区为主,在MRI图像中,脑血管周围间隙扩大呈圆形、椭圆形、蛛网状,无占位效应,其T1WI序列和T2WI序列均可见扩大间隙信号,T2WI-FLAIR序列等或稍低信号,增强扫描未见强化。结论:脑血管周围间隙扩大(Enlarged perivascular Spaces,EPVS)在MRI图像中具有典型表现,MRI可对脑血管周围间隙扩大予以准确诊断,具有较高的诊断价值。
Objective: To investigate and evaluate the value of magnetic resonance imaging (Magnetic, resonance, imaging, MRI) in the diagnosis of enlargement of cerebral perivascular space. Methods: in our hospital in January 2015 ~ 2016 year in December 100 cases there may be enlarged perivascular spaces in patients with type 2 diabetes as the research ob- ject, all the patients underwent MRI examination, routine T1WI, T'2WI and T2WI- FLAIR sequence scanning, part of injec- ted contrast agent enhanced scanning, the MRI images were observed, MRI analysis cerebral perivascular space expansion, to make the diagnosis, the diagnostic accuracy of MRI on cerebral perivascular space expansion. Results: of the 100 patients, the diameter of the cerebral perivascular space was more than 3mm in 43 patients. It was diagnosed that the space around the cerebral vessels was widened, and the diagnostic accuracy of MRI in the widening of the space around the cerebral vessels was 100%. 43 cases of cerebrovascular patients diagnosed around the gap were located in the cerebral hemispheres, the basal gan- glia and corona radiata, in the MRI image, cerebral perivascular space expansion were round or oval, spider, no space occup- ying effect, the T1WI and T2WI sequences were found to expand the gap signal, T2WI -FLAIR sequence or slightly low sig- nal, enhanced scan showed no enhancement. Conclusion: Enlarged perivaseular, Spaces, EPVS) showed typical features in MRI images, and MRI could accurately diagnose the enlargement of the space around the cerebral vessels, so it has high diag- nostic value.
