腰椎间盘突出MRI和CT诊断的价值分析

Value of MRI and CT in the Diagnosis of Lumbar Disc Herniation

目的:分析腰椎间盘突出行MRI和CT诊断的价值。方法:分析本院于2016年3月-2017年3月收治的腰椎间盘突出120例患者资料,按照诊断方法不同分成两组,将行CT诊断60例患者作为对照组,将行MRI诊断60例患者作为观察组,对两组诊断价值进行比较。结果:CT影像学特征表现为:边界模糊、后缘锐角、髓核突出或异位;MRI影像学特征表现为:髓核异位、边缘不规则或见低信号;观察组于脊髓变形、椎间盘变形和硬膜囊受压检出率45.00%、25.00%和58.33%均比对照组16.67%、10.00%和33.33%高,且诊断总符合率96.67%比对照组76.67%高(P<0.05)。结论:腰椎间盘突出行MRI和CT诊断均具较高价值,临床需合理利用,以进一步提高疾病诊断准确率,从而为临床尽早确诊和采取有效措施提供参考。

Objective： To analyze the value of MRI and CT in the diagnosis of lumbar disc herniation. Methods： The clinical data of 120 patients with lumbar disc herniation who had been seeking treatment in the hospital between March 2016 and March 2017 were analyzed. According to the different diagnostic methods applied, these patients were divided into 2 groups ： the 60 cases given CT diagnosis were taken as the control group, whereas the other 60 given MRI for diagnosis as the observation group. The diagnostic values were compared between the two groups. Results： The CT imaging features were： blurred boundary, acute posterior edge, prominent or ectopic nucleus pulposus; The MRI imaging features were characterized by： Ectopic nucleus, irregular edge or low signal. The detection rates of spinal cord deformation, intervertebral disc deforma- tion and dural compression in the observation group were respectively 45. 00%, 25.00% and 58. 33%, all higher than those in the control group, which were respectively 16. 67%, 10. 00% and 33.33%, and the total diagnostic coincidence rate was 96. 67% in the observation group, higher than the 76. 67% in the control group （P 〈0. 05）. Conclusion： For patients with lumbar disc herniation, both MRI and CT are with relatively high diagnostic value. In clinical practice, these two could be used to further improve the accuracy of diagnosis, to provide reference for early diaznosis and effective measures.
