摘要
目的探讨老年2型糖尿病患者中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)与糖尿病肾病的相关性。方法219例老年2型糖尿病患者根据尿微量蛋白排泄率(UAER)分为糖尿病肾病组(DN组)110例和无糖尿病肾病组(DM组)109例,另选同期进行体检的健康老年人101例作为正常对照组(NC组)。收集三组一般临床资料及生化指标,分析NLR、PLR在各组间的差异及与DN的相关性。结果DM组和DN组尿素氮(BUN)、尿酸(uA)、胱抑素-C(Cys-C)、三酰甘油(TG)、糖化血红蛋白(HbAle)均高于NC组,高密度脂蛋白胆回醇(HDL-C)低于NC组,差异有统计学意义(P〈0.05);DN组BUN、肌酐(Cr)、UA、Cys-C、β2-微球蛋白(β2-MG)、1-谷氨酰转肽酶(GGT)、TC、TG、低密度脂蛋白胆固醇(LDL—C)、HbAlc、NLR、PLR高于DM组,差异有统计学意义(P〈0.05)。Pearson相关分析显示,NLR与BUN、Cr、UA、Cys.C呈正相关(r=0.388、0.302、0.260、0.365,P〈0.05),与HDL—C呈负相关(r=-0.104,P〈0.05);PLR与cr、UA、Cys-C、β2-MG呈正相关(r=0.138、0.237、0.180、0.133,P〈0.05)。结论NLR、PLR与他DM患者DN的发生密切相关,可能成为预测DN发生的新指标。
Objective To investigate the correlation of neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) and diabetic nephropathy in elderly patients with type 2 diabetic mel- litus (T2DM). Methods A total of 219 T2DM elderly patients were divided into diabetic nephropathy group (DN group, n = 110) and non-diabetic nephropathy group (DM group, n = 109) according to the urinary albumin excretion rates (UAER). Meanwhile, 101 healthy elderly people who underwent physi- cal examination were selected as the normal control group (NC group). Clinical and biochemical data were collected to compare the differences in NLR and PLR among the three groups and the relationship between NLR, PLR and DN was analyzed. Results Compared with NC group, the levels of blood urea nitrogen ( BUN), uric acid ( UA), Cystatin-C ( Cys-C ), triglyceride ( TG), glyeosylated hemoglobin (HbA1 c) were higher, while the level of high-density lipoprotein cholesterol (HDL-C) was lower in DM and DN groups, the differences were statistically significant ( all P 〈 0. 05). The levels of BUN, creati- nine (Cr), UA, Cys-C, β-2 microglobulin (β2-MG) γ-glutamyl-transpeptidase (GGT), TC, TG, LDL-C, HbAlc, NLR, PLR were higher in DN group than those in DM group, the differences were sig- nificant ( all P 〈 0.05 ). Pearson correlation analysis showed that the level of NLR was positively correla- ted with BUN, Cr, UA and Cys-C (r = 0. 388, 0. 302, 0. 206, 0. 365, P 〈 0.05) , and negatively corre- lated with HDL-C ( r = - 0. 104, P 〈 0. 05 ). PLR level was positively correlated with Cr, UA, Cys-C and β2-MG (r =0. 138, 0. 237, 0. 180, 0. 133, P 〈0.05. Conclusions The levels of NLR, PLR are significantly associated with DN in patients with T2DM. High levels of NLR, PLR may be reliable predic- tive makers for DN.
