套筒冠义齿修复对老年牙周炎伴牙列缺损患者牙齿松动度改善及生活质量的影响

Effect of telescopic denture restoration on improvement of tooth loosening and quality of life in elderly patients with periodontitis and dentition defect

目的探讨套筒冠义齿修复对老年牙周炎伴牙列缺损患者牙齿松动度改善及生活质量的影响。方法72例老年牙周炎伴牙列缺损患者通过随机数字表法分为研究组和对照组，每组36例。对照组采用常规可摘普通义齿修复，研究组采用套筒冠义齿修复。6个月后进行随访，统计对比两组治疗前后牙齿松动情况、生活质量评分变化情况、咀嚼功能治疗效果、牙周相关指标[出血指数（SBI）、牙龈指数（GI）、探诊深度（PD）]变化情况。结果①牙齿松动情况：治疗前两组牙齿松动度比较差异未见统计学意义（P〉0．05）；治疗后，两组牙齿松动度比较差异有统计学意义（P〈0．05）。②咀嚼功能：研究组咀嚼功能治疗效果优于对照组，差异有统计学意义（P〈0．05），且研究组显效率[52．78％（19／36）]高于对照组[33．33％（12／36）]。③生活质量：治疗前两组生理功能、总体健康、躯体疼痛、社会功能、精神健康、活力评分比较差异未见统计学意义（P〉0．05）；治疗后，研究组各维度分值均高于对照组，差异有统计学意义（P〈0．05）。④牙周情况：治疗前两组SBI、GI、PD比较差异未见统计学意义（P〉0．05）；治疗后，研究组SBI、GI、PD低于对照组，差异有统计学意义（P〈0．05）。结论采用套筒冠义齿修复治疗老年牙周炎伴牙列缺损效果显著，可有效改善患者牙齿松动度、牙周情况与生活质量，提高牙齿咀嚼功能。，

Objective To investigate the effect of telescopic denture restoration on improvement of tooth loosening and quality of life in elderly patients with periodontitis and dentition defect. Methods Seventy-two elderly patients with periodontitis and dentition defect were divided into control group and study group according to the random table method, with 36 cases in each group. The control group was treated by conventional removable normal denture, the study group was treated by telescopic denture. Patients were followed up after six months. Loosening of teeth, quality of life scores, masticato- ry treatment effect, periodontal related indicators [ sulcus bleeding index （SBI）, gingival index （GI）, probing depth （PD） ] were compared before and after treatment. Results （1）Loose teeth situation： there was no significant difference in the degree of tooth loosening between the two groups before treatment （P 〉 0.05）, however, there was significant difference between the two groups after treatment （P 〈 0. 05）. （2）Chewing function： the curative effect of study group was better than that of control group, the difference was statistically significant （P 〈 0.05 ）, and the effective rate of study group [ 52.78% （19/36） ] was higher than that of control group [33.33% （12/36） ]. （3）Quality of life： there were no significant difference in the scores of physiological function, general health, physical pain, social func- tion, mental health and vitality score between the two groups before treatment （ all P 〉 0.05） , but after treatment, the scores of study group were higher than those of control group, the differences were statisti- cally significant （ all P 〈 0.05 ）. （4）Periodonta] situation ： there was no significant difference in SBI, GI and PD between the two groups before treatment （P 〉 0.05） , but after treatment, SBI, GI and PD instudy group were lower than those in control group, the differences were statistically significant （ all P 〈 0.05）. Conclusions Telescopic denture restoration gets significant effect on the treatment of elderly pa- tients with periodontitis and dentition defect, which can effectively improve the patients＇ tooth loosening, periodontal condition and quality of life and improve the teeth chewing function.
