摘要
目的探讨多次单采血小板献血者体内储存铁的变化情况。方法选择捐献血小板≥16次且捐献量≥29 U的献血者99名(男84名、女15名)作为实验组,随机选择首次仅捐献全血献血者100名(男86名、女14名)作为对照组,检测其可溶性转铁蛋白受体(s TfR)、血清铁蛋白(SF)的水平;应用双因素方差分析方法对2组检测结果做统计学分析,并按献血者捐献血小板次数及性别做分类统计。结果 s TfR(mg/L):男性及女性捐献血小板组和未捐献血小板组分别为2.877±1.245 vs 2.614±0.706及3.029±0.908 vs 2.453±0.769(P<0.05),捐献50-100次组与捐献0次组分别为3.031±1.361 vs 2.591±0.713(P<0.01);SF(ng/m L):男性及女性捐献血小板组和未捐献血小板组分别为45.89±35.67 vs 105.35±91.64及29.40±24.19 vs 91.32±68.97(P<0.01),捐献16-50次组>50-100次组及≥101次组分别为42.41±3.90,41.99±34.84,47.23±35.68,而捐献0次组为103.39±88.65(P<0.01);性别对s TfR和SF的影响比较(P>0.05)。结论多次单采血小板献血者SF下降、s TfR升高,但变化均处在参考值范围以内,说明多次捐献血小板是安全的,但仍需应引起关注。
Objective To explore the change of reserve ferrum of in blood donors with multiple platelet apheresis.Methods 99 donors( 84 males and 15 females) who donated platelets for equal or more than 16 times and 29 U were selected as the experimental group. 100 whole blood donors( 86 males and 14 females) were randomly selected as the control group. The soluble transferrin receptor( s TfR) and serum ferritin( SF) levels were measured. The results in the two groups were analyzed with double factor analysis of variance. Statistical classification was performed based on platelet apheresis duration and gender. Results s TfR( mg/L) : Male and female platelet donors and the control group presented concentrations of respectively 2. 877±1. 245 vs 2. 614±0. 706 and 3. 029±0. 908 vs 2. 453±0. 769( P〈0. 05). The donors who donated platelets for 50-100 times and 0 time were measured at 3. 031± 1. 361 vs 2. 591± 0. 713,respectively( P〈0. 01). SF( ng/m L) : Male and female platelet donors and the control group presented concentrations 45. 89±35. 67 vs 105. 35±91. 6 and 29. 40±24. 19 vs91. 32±68. 97,respectively( P〈0. 01). The donors who donated platelets 16-50 times,50-100 times and ≥101 time were measured at 42. 41±3. 90,41. 99±34. 84,47. 23±35. 68,respectively. Donors who never donated platelets were measured at103. 39±88. 65( P〈0. 01). Effect of gender on s TfR and SF was minor( P〉0. 05). Conclusion Multiple platelet apheresis may result in decreased SF and increased s TfR. This,however,is still within the reference range. Our study indicates that it is safe to donate platelets many times. That being said,we need to pay closer attention to the issue and try to minimize potential threats.
