社区糖尿病患者三级预防管理中专科引领下的“糖尿病联盟”干预成效调查研究

Investigation of and Research on the Effect of "Diabetes Alliance" Intervention Under the Specialist in Tertiary Management and Prevention for Community Diabetics Patients

目的探讨由糖尿病专科引领下的"糖尿病联盟"在社区糖尿病患者三级预防管理中的相关性干预成效。方法采用资料回顾调查分析法,选取2016年1—10月经前期流调筛选入组的532例社区糖尿病患者,由连云港市赣榆区人民医院糖尿病专科引领下成立的"糖尿病联盟"进行为期1年的三级预防多途径管理系列干预,对比研究组糖尿病患者干预前后的平均血糖监测水平、自我管理能力、糖尿病相关知识知晓率、依从性及并发症发生情况。结果研究组患者干预后血糖监测平均水平低于干预前(P<0.05);干预后的糖尿病患者自我管理能力、糖尿病相关知识知晓率、依从性高于干预前(P<0.05)。结论由糖尿病专科引领下的"糖尿病联盟"能发挥专科医疗特色,促进社区糖尿病患者三级预防管理工作的推进,提高患者自我管理能力和依从性,减少并发症的发生。

Objective To explore the effect of "diabetes alliance" intervention under the specialist in tertiary management and prevention in community diabetics. Methods 532 community diabetic patients who were screened from prophase epidemiological investigation from January to October 2016 were selected with data review survey analysis method and multi-channel management of tertiary prevention series of intervention was carried out by "Diabetes Alliance" led by diabetes department of Ganyu District People's Hospital of Lianyungang City for one year. The average level of blood glucose monitoring, self-management ability, awareness of diabetes-related knowledge, compliance and complications before and after intervention in the study group were compared. Results The mean level of blood glucose monitoring after intervention in study group was significantly lower than that before intervention(P<0.05). After intervention, the ability of self-management, awareness of diabetes-related knowledge, compliance of diabetic patients was significantly higher than that before intervention(P<0.05). Conclusion Under the guidance of diabetes specialist, the "Diabetes Alliance" can play a special medical features, significantly promote prevention and treatment of community diabetic patients, improve patients' self-management and compliance, reduce complications
