前列地尔对慢性充血性心力衰竭患者心功能和血管内皮功能的影响

Influence of alprostadil on the cardiac and blood vessel endothelial function of patients with chronic congestive heart failure

目的探讨前列地尔对慢性充血性心力衰竭(CHF)患者心功能和血管内皮功能的影响。方法选择慢性CHF患者126例,按照随机原则分为对照组与研究组,每组各63例。两组患者均采取慢性CHF常规治疗,研究组在此基础上联合前列地尔注射液治疗,疗程均为14 d。观察并对比两组临床疗效,以及治疗前、治疗后患者心功能、血清内皮素-1(ET-1)与一氧化氮(NO)变化。结果研究组总有效率显著高于对照组(P<0.05)。治疗后研究组左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)显著降低,左室短轴收缩短率(FS)及左室射血分数(LVEF)显著升高,研究组LVESD、LVEDD显著低于对照组,FS、LVEF显著高于对照组(P<0.05)。治疗后研究组血清ET-1显著降低,NO显著升高,研究组血清ET-1显著低于对照组,NO显著高于对照组(P<0.05)。结论前列地尔治疗慢性CHF疗效较好,可有效增强患者的心功能,改善血管内皮功能。

Objective To explore the influence of alprostadil on the cardiac and blood vessel endothelial function of patients with chronic congestive heart failure(CHF).Me thods 126 cases of CHF patients were randomly divided into the control group and the research group,with 63 cases in each group.Both groups were given routine CHF treatment for 14 days.The research group was given alprostadil injection in addition with a treatment course of 14 days.The clinical effect,the cardiac function before and after the treatment,the changes of serum endothelin-1(ET-1) and nitric oxide(NO) of the two groups were observed and compared.Re s ults The total effective rate of the research group was significantly higher than that of the control group( P <0.05).After treatment,the left ventricular systolic end-stage internal diameter(LVESD),left ventricular diastolic internal diameter(LVEDD) of the research group decreased significantly while the shortening of left ventricle short axis(FS) and the left ventricular ejection fraction(LVEF) increased significantly.LEVSD and LVEDD of the research group were significantly lower than those of the control group while FS and LVEF of the research group were significantly higher than those of the control group( P<0.05).After treatment,ET-1 of the research group significantly decreased while NO increased significantly.ET-1 of the research group was significantly lower than that of the control group while NO was significantly higher than that of the control group(P <0.05).Conclus ion Alprostadil enjoyed a good curative effect in the treatment of chronic congestive heart failure patients,improving their cardiac and blood vessel endothelial functions.
