骨科康复一体化模式对老年股骨粗隆间骨折术后功能恢复影响的研究

Influence study of integrated orthopaedic rehabilitation on function recovery of the elderly undergoing femoral trochanter fracture operation

目的评价骨科康复一体化治疗模式运用于老年股骨粗隆间骨折术后髋关节疼痛、功能的改善效果,比较其与常规治疗模式干预的康复疗效差异。方法将2014年1月至2016年1月来天津疗养院骨科就诊,诊断为股骨粗隆间骨折患者60例,AO分型为A型,按就诊顺序随机分为观察组(30例)和对照组(30例),观察组给予骨科康复一体化治疗模式干预,对照组给予常规治疗模式干预,分别于术前和术后1周、2周、4周、12周、24周采用视觉模拟量表(visual analogue scale,VAS)评分、Harris髋关节功能评分评价康复疗效。结果术前两组患者VAS评分无差异(P>0.05);术后1、2周两组VAS评分、Harris髋关节评分两组无差异(P>0.05);术后4周、12周及24周时观察组VAS评分、Harris髋关节功能评分均明显优于对照组(P<0.05)。结论对初次股骨粗隆间骨折的患者进行骨科康复一体化治疗模式干预后的康复疗效确切,其对改善缓解疼痛及髋关节功能的康复疗效优于常规的治疗模式。

Objective To evaluate the effect of integrated orthopaedic rehabilitation in the treatment of hip joint pains and function improvement for the elderly after femoral trochanter fracture operation.Me thods 60 cases of patients with femoral trochanter fracture admitted by the orthopedics department of Tianjin Sanatorium from January2014 to January 2016 were selected as the research subjects and randomly divided into the observation group and the control group,with 30 cases in each.Their AO classification was A.The observation groups was given integrated orthopaedic rehabilitation while the control group was given routine treatment.Visual analogue scale(VAS) and Harris hip scores were adopted to evaluate the rehabilitation effect before operation,1 week,2 weeks,4 weeks,12 weeks and 24 weeks after operation.Re s ults Before operation,VAS scores of patients in both groups showed no difference( P>0.05).1 week and 2 weeks after operation,VAS and Harris hip scores of both groups showed no difference( P >0.05).4 weeks,12 weeks and 24 weeks after operation,VAS and Harris hip scores of the observation group were significantly better than those of the control group( P <0.05).Conclus ion Integrated orthopaedic rehabilitation had a definite effect on patients inflicted with femoral trochanter fracture for the first time.Its rehabilitation effects on relieving pains and improving hip joint function were better than those of routine treatment.
