血清胱抑素C与肾功能损害的关系及血清胱抑素C对早期高血压肾损害患者的临床观察分析 被引量:12

摘要 目的:探讨血清胱抑素C(cystatin C)作为早期肾功能损害及肾损伤时敏感标志物,其数值的高低与肾功能的关系及对早期高血压肾损害患者临床观察的意义。方法:采用乳胶颗粒增强免疫比浊法(PENIA)检测患者Cys C,同时检测尿素氮(BUN)、血肌酐(Scr),并根据血清胱抑素C(cystatin C)数值给予分组,0.00~1.16 mg/L(A组),1.17~2.00 mg/L(B组),2.00~3.00 mg/L(C组),>3.00 mg/L(D组)。3 133例检查患者中确诊的高血压患者617例A组(378例)、B组(181例)、C组(32例)、D组(26例),根据尿微量白蛋白排泄率(UAER)>30 mg/24 h,查看各组构成比。通过对617例高血压患者的临床资料分析,以得出血清胱抑素C对早期高血压肾损害患者的临床观察的意义。结果:随着Cys C数值的不断升高,尿素氮、肌酐异常患者的百分比和其血清浓度不断上升。且随着Cys C浓度升高,高血压肾功能损害患者所占比例越高。结论:Cys C在高血压肾病早期有较高的准确性,在高血压肾病早期的诊断具有重要的意义,且血清浓度的越高,肾功能损害越严重,结合血清肌酐、尿素氮等可以更准确判断肾小球滤过功能,Cys C可作为临床早期高血压病肾病诊断及其严重程度的可靠指标。 Objective Urinary inhibition to explore serum cystatin C as early kidney damage and renal damage sensitive marker,The relationship between the level of the value and the function of the kidney and the clinical observation of early hypertensive renal damage. Method Using latex particles enhance immune turbidimetric method(PENIA) detection in patients with Cys C,at the same time testing urea nitrogen(BUN),serum creatinine(Scr),and according to the serum cystatin C numerical C group,0. 00 - 1. 16 mg/L(group A),1. 17 - 2. 00 mg/L(group B),2. 00 - 3. 00 mg/L(group C), 3. 00/L(group D). 3133 cases of in patients with the diagnosis of 617 cases of hypertension patients A(378 cases) and B(181 cases),C(32 cases),D(26 cases),according to the urine trace albumin excretion rate(UAER) 30 mg/24 h,each constituent ratio was checked. Through the analysis of the clinical data of 617 cases of patients with hypertension,with serum cystatin C in patients with hypertensive renal damage to observe the clinical significance of early. Results With the constant rise of Cys C numerical,urea nitrogen,creatinine,the percentage of patients with abnormal serum and its concentrations rising. As Cys C concentrations rise,and the higher the proportion of patients with hypertensive renal impairment. Conclusion Cys C have higher accuracy in the early high blood pressure,kidney disease,the high blood pressure,kidney disease early diagnosis is of great importance,and the higher the concentration of serum,renal damage,combined with serum creatinine and urea nitrogen can more accurately determine glomerular filtration function,Cys C can be used as a clinical early hypertension kidney disease diagnosis and the reliability index of the severity.
