泉州市2009年～2016年孕产妇死亡情况分析

Analysis on the Maternal Mortality in Quanzhou during the Period from 2009-2016

目的:了解泉州市2009年~2016年孕产妇死亡率、孕期保健情况及死亡原因,分析影响因素,为进一步降低孕产妇死亡率提供依据。方法:采用回顾性分析方法,分析2009年~2016年泉州市孕产妇死亡报告卡及个案调查报告、市级评审结果、WHO十二格表。结果:常住人口孕产妇死亡率为8.75/10万,呈下降趋势,外来人口孕产妇死亡率为36.54/10万,波动较大;死亡孕产妇平均年龄为29.61岁,文化程度初中及以下占76.53%,外来户口与常住户口在孕产次、孕产期保健情况上有明显差异;直接死亡原因占46.38%,间接产科原因占53.62%;WHO十二格表结果显示孕产妇死亡主要与个人家庭的知识技能(51.28%)、医疗保健的知识技能(41.03%)及个人家庭的态度(16.67%)有关。结论:加强流动人口孕产妇系统管理,强化孕前保健的重要,提高医护人员专业技术水平及完善危重症孕产妇转诊网络是降低孕产妇死亡的重要措施。

Objective To understand the maternal mortality,pregnancy healthcare and death causes in Quanzhou during the period from 2009-2016 and analyze the affecting factors,thereby providing a basis for further reducing the maternal mortality. Method Using the retrospective analysis method to analyze the maternal death report card,case investigation reports,city review results and WHO maternal death causes form in Quanzhou during the period from 2009 - 2016. Results Maternal mortality of resident population was dropping,recorded at 8. 75/100,000 while that of floating population is fluctuating significantly,recorded at 36. 54/100,000; dead pregnant and lying-in women has an average age of 29. 61 years old and 76. 53% of them have an education qualification of junior middle school and lower. Floating population and resident population were significantly different in terms of gravidity and parity history and maternal healthcare; direct death causes account for 46. 38% while indirect obstetric causes account for 53. 62%; according to WHO maternal death causes form,maternal mortality was mainly related to knowledge skills of personal family（51. 28%）,knowledge skills regarding healthcare（41. 03%） and attitude of personal family（16. 67%）. Conclusion Strengthening the maternal system management of floating population,emphasizing the importance of pre-pregnancy healthcare,improving the professional technique of healthcare staff,and perfecting the referral network for critical pregnant and lying-in women are important measures to reduce the maternal mortality.
