The Study of “Internet plus” Health Management Model for Patients with Hypertension

高血压是一种常见、危害较大的疾病,传统的"医生-患者"管理模式由于管理单一、患者不配合等诸多因素影响,难以有效控制该类疾病,给社会和个人带来很多不确定风险。文章利用互联网技术,在健康团队与患者之间建立一个有效的健康管理沟通平台,该平台通过收集高血压居民的健康信息,给予患者日常生活、用药指导、健康教育等综合健康指导。社区高血压居民健康管理实践表明:"互联网+"高血压患者健康管理模式能有效地控制居民血压水平,提升居民生活质量。

Hypertension is a common and harmful disease,the traditional " Doctor-patient" management mode can not effectively control the disease due to various reasons such as patients' incompliance and single management,which brings a lot of uncertain risks to the society and people. In this paper,by using internet technology,we established an effective health management communication platform between the health team and the patients. Based on this platform,we collected the health information of hypertension,gave comprehensive health patients in daily life,medication guidance,health education and guidance. The practice showed that the "Internet plus"health management of hypertension patients could effectively control the blood pressure levels of residents and enhance the quality of life of residents.
