脾脏转移性绒毛膜癌伴脾破裂1例报道并文献复习

A Case Report of Metastatic Choriocarcinoma with Rupture of Spleen and Review of the Literature

目的:探讨脾脏转移性绒毛膜癌伴脾破裂出血的临床特点、诊断及治疗。方法:分析笔者所在医院近期对1例脾脏转移性绒毛膜癌伴破裂出血患者的诊治情况,并复习相关文献。结果:术中发现患者脾脏下极有一包块且破裂出血,术后病理诊断脾脏绒毛膜癌,血β-h CG 121 475 IU/L。胸部CT示两肺转移瘤。结论:临床上脾脏转移性绒毛膜癌伴破裂出血在脾破裂中极为少见,对于脾脏占位,尤其是处于生育年龄的女性患者,应注意该病的鉴别诊断。

Objective： To investigate the clinical manifestation, disagnosis and treatment of metastatic choriocarcinoma with rupture of spleen. Method： Aanalyzing the process of therapy for the patient diagnosed with metastatic choriocarcinoma with rupture of spleen, and reviewing the related literature. Result： We found a tumor with rupture and bleeding in the bottom of spleen during operation, postoperative pathological reported choriocarcinoma of spleen, the β -hCG was elevated at 121 475 1U/L, and the chest CT reported multimetastatic carcinomas of lungs.Conclusion： Metastatic choriocareinoma with rupture of spleen in splenic rupture is rare inclinical medicine, and it shall be paid attention to the differential diagnosis of splenic tumors, especially for the female patients of child-bearing age.
