脑卒中家庭肠内营养患者主要照顾者负担现状及影响因素

Burden status quo and influencing factors of main caregivers in stroke patients with home enteral nutrition

目的调查脑卒中肠内营养患者在行家庭肠内营养期间照顾者的负担现状,分析照顾者负担的影响因素。方法收集在该院住院行肠内营养治疗、且出院后继续实施家庭肠内营养治疗的脑卒中患者的直接照顾者作为研究对象共202人。采用照顾者负担问卷、焦虑自评量表、抑郁自评量表、患者及其照顾者一般情况进行问卷调查,统计分析照顾者负担水平状况及其相关影响因素。结果结果显示202名接受调查的照顾者负担总评分(50.17±9.75)分。单因素分析结果显示照顾者的年龄、家庭人均收入水平、照顾者是否伴有负性情绪(焦虑或抑郁)、照顾时间、患者NIHSS评分及喂养方式的负担评分差异有统计学意义(P<0.05)。多因素分析结果显示照顾者照顾时间、喂养方式、照顾者伴有焦虑或伴有抑郁、患者NIHSS评分是照顾者负担的主要影响因素(P<0.05)。结论照顾者在照顾患者时存在较大的负担,应当引起关注。

Objective To investigate the burden status quo of careglvers in stroke patients with home enteral nutrition and to analyze the factors influencing the caregivers burden. Methods A total of 202 direct caregivers in stroke patients with enteral nutrition treatment during hospitalization period in this hospital and continued home entera] nutrition after discharge from hospital were collected as the study subjects. The Caregiver Burden Inventory, Self--Rating Anxiety Scale, Self-- Rating Depression Scale, patients and caregivers general condition questionnaire were adopted to conduct the survey. The burden level status of caregivers and its related influencing factors were statistically analyzed. Results The results showed that the burden total score in 202 caregivers receiving the investigation was （50.17± 9.75） points. The univariate analysis results showed that that the age of caregivers, family income per capita level,whether earegivers having negative emotions （anxiety or depression）, care time, NIHSS scores of patients and feeding mode burden score had statistical differences （P〈0.05）. The multivariate analysis results showed that the care time of caregivers, feeding mode, accompanying anxiety or depression in caregivers, NIHSS scores of patients were the main influencing factors of caregivers burden（P〈0.05）. Conclusion The caregivers have a great burden m caring the patients,which should arouse attention.
