摘要 目的研究BI-RADS分级在彩超检查乳腺良恶性肿瘤中的规范化应用价值。方法选取2016年1月—2017年7月接收的乳腺疾病且将接受外科手术治疗患者30例,采取高频彩超检测患者双侧乳房,且根据BI-RADS对患者乳腺病灶分级,选择BI-RADS分级为3~5级的26例患者,将BI-RADS分级结果与病理结果行对比分析,判定BI-RADS分级于乳腺良恶性肿瘤鉴别的应用效果。结果乳腺病灶BI-RADS分级3~5级相关因素间对比,差异有统计学意义(P<0.05);病灶BI-RADS分级3级~4a级中病理学诊断结果良性10例(90.91%),恶性1例(9.09%);病灶BI-RADS分级4b级及以上中病理学诊断结果良性2例(20.00%),恶性8例(80.00%);病灶BI-RADS分级5级中病理学诊断结果良性0例(0.00%),恶性5例(100.00%)。BIRADS分级诊断敏感度达92.86%,特异度达83.33%。结论乳腺病灶BI-RADS分级检测乳腺良、恶性肿瘤的敏感度及特异度较高,在高频彩超检查乳腺状况时应用BI-RADS分级对乳腺癌的诊断及治疗方案具有重要的指导意义。 Objective To study the standardized application value of BI-RADS grading in color doppler ultrasound examination of breast benign and malignant tumors. Methods 30 cases with breast disease undergoing surgical treatment from January 2016 to July 2017 were selected. High frequency color doppler ultrasound was used to detect the patients' bilateral breasts, and the breast lesions were classifed according to BI-RADS. 26 patients with BI-RADS grade 3 to 5 were selected, and the results of BI-RADS classification with the pathological results were compared. To determine the application of BI-RADS classification in the differential diagnosis of benign and malignant breast tumors. Results Grade 3 to 5 level factors of the BI-RADS classification of breast lesions were compared, the difference was statistically significant (P 〈 0.05). The pathological diagnosis of BI-RADS grade 3 to 4a was benign in 10 cases (90.91%) and malignant in 1 case (9.09%), BI-RADS grade 4B and above was benign in 2 cases (20%) and malignant in 8 cases (80%), BI-RADS grade 5 was benign in 0 cases (0%) and malignant in 5 cases (100%). The sensitivity of BI-RADS grading diagnosis was 92.86% and the specifcity was 83.33%. Conclusion There is a high sensitivity and specificity in detecting breast lesions by BI-RADS classification. The application of BI-RADS grading is of great guiding signifcance in the diagnosis and treatment of breast cancer.
