长期小剂量克拉霉素联合鼻用激素及鼻腔冲洗对尘肺与无尘肺慢性鼻窦炎患者的疗效比较

Curative effect comparison of long-term low-dose of clarithromycin combined with nasal hormone and nasal douche in CRS patients with or without pneumoconiosis

目的比较克拉霉素联合鼻用激素及鼻腔冲洗对尘肺与无尘肺慢性鼻窦炎患者的疗效。方法选取慢性鼻窦炎伴尘肺患者42例(尘肺组)与无尘肺患者45例(无尘肺组),均给予长期小剂量克拉霉素联合鼻用激素及鼻腔冲洗治疗。比较两组患者疗效。结果治疗前,两组患者VAS评分及鼻总阻力均无明显差异(P>0.05);治疗后,无尘肺组流涕、鼻塞、头痛、嗅觉VAS评分明显优于治疗前与尘肺组(P<0.05);两组患者鼻总阻力均改善(P<0.05),但组间差异不明显(P>0.05)。尘肺组患者治疗总有效率低于无尘肺组(P<0.01)。结论克拉霉素联合鼻用激素及鼻腔冲洗治疗尘肺患者慢性鼻窦炎的效果较差,提示尘肺患者病变较严重。

Objective To compare the therapeutic effect of clarithromycin combined with nasal hormone and nasal douche in chronic rhinosinusitis（CRS） patients with or without pneumoconiosis. Methods Fourty-two CRS patients with pneumoconiosis（pneumoconiosis group） and 45 cases without pneumoconiosis（without pneumoconiosis group） were included, the patients were all treated with long-term low-dose of clarithromycin combined with nasal hormone and nasal douche.The clinical effects of the two groups were compared. Results Before treatment, there were no significant differences in VAS scores and total nasal resistance between the two groups（P〉0.05）; after treatment, the VAS scores of nasal discharge,nasal obstruction, headache and olfactory sensation in the without pneumoconiosis group were better than those before treatment and in the pneumoconiosis group（P〈0.05）; the total nasal resistance improved in both groups（P〈0.05）, but the difference between the two groups was not significant（P〉0.05）. The total effective rate of treatment of the without pneumoconiosis group was lower than that of the pneumoconiosis group（P〈0.01）. Conclusion The effect of this method in CRS patients with pneumoconiosis was poorer, which is suggested that the CRS patients with pneumoconiosis are more serious.
