摘要
(1)目的观察不同剂量聚乙二醇电解质散(PEG)溶液行全结肠镜检查肠道准备效果。(2)方法采用单盲、前瞻性研究方法,剔除因患者不能耐受、肠腔狭窄等结肠镜检查未能达于回盲部的病例7例,纳入分析143例,将其分为两组,A组75例,B组68例。A组结肠镜检查当天口服2LPEG溶液,B组分次口服4LPEG溶液。肠道清洁度采用波士顿评分量表(Boston bowel preparation scale,BBPS)评估;记录结肠镜检查进镜时间及是否检出大肠息肉;记录患者肠道准备过程耐受性及不良反应情况。(3)结果两组肠道清洁效果比较,综合评分及右半结肠评分B组效果较A组好,差异有统计学意义(P<0.05);对于便秘患者来说,B组BBPS评分总分、左半结肠、中段结肠及右半结肠评分均较A组好(P<0.05);对于有腹部手术史者两组BBPS评分总分及右半结肠评分差异有统计学意义(P<0.05);B组发生呕吐、腹胀的几率要高于A组,差异有统计学意义(P<0.05);在进镜时间及息肉检出率方面两组差异无统计学意义(P>0.05)。(4)结论当天口服2LPEG溶液适用于腹泻或正常便者,分次口服4L PEG溶液适用于便秘者、有腹部手术史者或怀疑病变位于右半结肠者。
Objective To observe the effect of different doses of polyethylene glycol electrolyte(PEG)solution on bowel preparation by colonoscopy.Methods Single-blind,prospective research method was conducted,there were 143 cases with colonoscopy,75 cases in group A and 68 cases in group B,which received 2 LPEG and 4 LPEG,respectively.Intestinal cleanliness was assessed using the Boston bowel preparation scale(BBPS),insertion time,colonic polyps,the tolerance and adverse reactions of patients during bowel preparation were recorded.Results Comparison of two groups of intestinal cleaning effect,the difference between the composite score and the right colon score was statistically significant(P <0.05),cleansing effect of group B was better than that of group A.For patients with constipation,the BBPS total score,the left colon,the middle colon and the right colon score of the group B were better than those of the group A(P <0.05).For patients with abdominal surgery,BBPS total score and right colon score between the two groups had statistical significance(P<0.05).The incidence of vomit and distension in group B were higher than that in group A(P <0.05).There were no significant difference between the two groups in the time of colonoscopy and the detection rate of polyps(P >0.05).Conclusion 2 LPEG is suitable for the people of diarrhea or normal stool,4 LPEG suitable for the people of constipation,abdominal surgery or suspected lesions in the right colon.
