二级综合医院脑卒中患者早期康复介入管理模式及影响因素分析

Early rehabilitation intervention management model and influencing factor analysis of stroke patients in secondary general hospital

目的分析二级综合医院脑卒中患者接受早期康复介入管理模式,并探索人口教育水平、经济水平、医疗保险类型、康复认知态度对患者接受早期康复介入管理治疗的影响。方法选取2014-03—2017-03在湖北航天医院住院的129例脑卒中患者,采取单双随机分组,观察组于发病病情稳定48h行康复治疗,对照组于病情稳定2周后行康复治疗,所有患者进行问卷调查。结果观察组较对照组神经功能缺损及日常生活指数改善明显;不同医疗保险类型、费用支付方式、收入来源,以及对制定康复治疗方案的方式有不同认识的患者,在脑卒中发作住院期是否接受早期康复治疗有明显差异。结论二级综合医院脑卒中患者早期康复介入管理模式可降低脑卒中神经功能缺损评分;年龄、文化程度、婚姻状况对接受早期康复介入管理无影响,而经济收入水平、医疗保险类型和康复认知态度等因素是影响脑卒中患者接受早期康复介入管理治疗的重要因素。

Objective To analyze the management mode of early rehabilitation intervention in patients with cerebral apoplexy in secondary general hospital, and to explore the effects of education level, economic level, medical insurance type and re- habilitation cognition on the therapeutic effects of this model. Methods Totally 129 cases of stroke patients in our hospital from March 2014 to March 2017 were divided into 2 groups, the observation group received rehabilitation treatment 48 hours after the onset, and the control group received rehabilitation treatment 2 weeks after the onset. All patients were surveyed by questionnaires. Results The patients in the observation group showed better neurologic function and daily life index than those of control group. The types of medical insurance, payment methods and income sources, the views on the way to formulate rehabilita- tion treatment plan could significant affect the acceptance of early rehabilitation treatment during the period of stroke. Conclusion The management mode of early rehabilitation interventional management in secondary comprehensive hospital can improve the neurologic function stroke patients. Age, cultural degree and marital status are not affected by early rehabilitation intervention management, while economic income level, medical insurance type and rehabilitation cognition attitude are important factors influ- encing the early rehabilitation intervention management in stroke patients.
