联合经腹与经阴道彩色多普勒超声在提高瘢痕妊娠诊断价值中的应用

Application of Abdominal Combined Vaginal Color Doppler Ultrasound for Improving the Diagnostic Value of Scar Pregnancy

目的:探讨联合经腹与经阴道彩色多普勒超声(简称彩超)在提高瘢痕妊娠诊断价值中的应用。方法:回顾性选取2014年6月至2017年6月长安医院204例瘢痕妊娠患者的经腹与经阴道彩超资料,以临床手术、病理结果为对照。结果:在诊断延伸型、孕囊型、包块型瘢痕妊娠的符合率及总符合率中,经腹彩超为70.00%、80.00%、77.78%、74.51%,经阴道彩超为80.00%、84.00%、83.33%、81.86%,联合时为97.00%、96.00%、96.30%、96.57%;在诊断延伸型瘢痕妊娠的符合率及总符合率中,联合时明显高于经阴道彩超,经阴道彩超明显高于经腹彩超(P<0.05),在诊断孕囊型、包块型瘢痕妊娠的符合率中,联合时明显高于经腹彩超、经阴道彩超(P<0.05)。结论:经腹、经阴道彩超对瘢痕妊娠具有一定的诊断价值,但对延伸型瘢痕妊娠诊断存在较大的误差,而联合时可有效提高其诊断价值。

Objective： To discuss the application of abdominal combined vaginal color doppler ultrasound for improving the diagnostic value of scar pregnancy. Methods： Transabdominal and transvaginal color ultrasonical examinations data of 204 patients with scar pregnancy were retrospective selected from June 2014 to June 2017 in our Hospital, which were controlled by clinical and pathological results. Results： In the diagnosis of extension, gestational cystic type, block type scar pregnancy coincidence rates and total coincidence rate, the abdominal color ultrasound were 70.00%, 80.00%, 77.78%, 74.51%, the vaginal color ultrasound were 80.00%, 84.00%, 83.33%, 81.86%, the combination were 97.00%, 96.00%, 96.30%, 96.57%. In the diagnosis of extension scar pregnancy coincidence rate and total coincidence rate, the combination were significantly higher than those of the vaginal color ultrasound, the vaginal color ultrasound were significantly higher than those of the abdominal color ultrasound （-P〈（）.05）, in the diagnosis ofgestational cystic type, block type scar pregnancy coincidence rates, the combination were significantly higher than those of the abdominal and vaginal color ultrasound （P〈0.05）. Conclusion： Abdominal and vaginal color ultrasound is a certain diagnostic value in the scar pregnancy, but they have some errors for expanded scar pregnancy, and the combination can effectively improve their diagnostic value.
