摘要 目的研究lgM抗体检测在儿科呼吸道感染性疾病中的临床应用价值。方法随机选择350例在本院接受治疗的患有呼吸道感染性疾病婴幼儿作为研究对象。采集所有患儿的血清,采用间接免疫荧光(IFA)法检测血清中的九项呼吸道感染病原体免疫球蛋白M(lgM)抗体,分析每项病原体阳性检测率,同时比较阳性检测率在不同性别、不同年龄患儿中的差异。且通过检测结果指导临床用药,评价lgM抗体检测方法的临床意义。结果 lgM抗体检测结果:阳性278例(79.4%),阴性72例(20.6%),其中,以肺炎链球菌(SPN)、肺炎支原体(MP)、肺炎衣原体(CP)、呼吸道合胞病毒(RSV)四项病原体阳性检测率较高,差异有统计学意义(P<0.05);在不同性别阳性检测率中,男性患儿为75.4%,女性患儿为73.6%,比较差异无统计学意义;在不同年龄段阳性检测率中,1~3岁患儿明显高于其他年龄段患儿,差异有统计学意义(P<0.05)。结论通过lgM抗体检测呼吸道感染性疾病患儿,检测结果快速,特异性强,可辅助指导临床合理用药,具有较高的临床应用价值。 Objective To study the clinical value of lgM antibody in children with respiratory tract infection diseases. Methods 350 cases were randomly selected in our hospital in treatment of respiratory tract infection. All children's serum were collected, and the nine immunoglobulin M(lgM) antibody were detected by indirect immunofluorescence(IFA), the positive rate of each pathogen in all patients was analyzed. At the same time, the positive detection rate in the different sex, and the children of different ages in the differences were compared. And the clinical medication were guided by using the test results to evaluate the clinical significance of lgM antibody detection. Results lgM antibody test results were positive in 278 cases(79.4%) and negative in 72 cases(20.6%). Among them, Streptococcus pneumonia(SPN), mycoplasma pneumonia(MP), Chlamydia pneumonia(CP) and Respiratory syncytial virus(RSV) were significantly higher than other pathogens, and the difference was statistically significant(P<0.05). And there were no significant differences between the male children(75.4%) and the female children(73.6%). In the positive rate of different age groups, children aged 1-3 years were significantly higher than those of other age groups, the difference was statistically significant(P<0.05). Conclusion The detection of lgM antibody in children with respiratory tract infection, the test results are fast and specific, can help guide the clinical rational use of drugs, and it has a high clinical value.
