通心络胶囊在不同中医证候脑梗死二级预防中的疗效比较研究

Comparative study of tongxinluo capsule with different TCM syndromes in the prevention of secondary prevention of cerebral infarction

目的:观察通心络胶囊在不同中医证候脑梗死二级预防中的应用效果。方法:选取200例急性脑梗死住院患者,其中中医辨证诊断为气虚血瘀证、阴虚风动证、风痰阻络证、风火上扰证各组为50例,随机每组各抽取10例作为常规治疗组,予以常规抑制血小板聚集、调脂等治疗。余160例在常规治疗下均给予通心络胶囊治疗,气虚血瘀组、阴虚风动组、风痰阻络组、风火上扰组各40例,总疗程为30天。观察比较各组临床症状改善情况,收集患者一般信息及疾病信息后进行评分,患者体征评分参照美国国立卫生研究所脑卒中评分(NIHSS评分量表)的评定标准,改良rankin量表(mRS评分量表)以及日常生活能力量表(BI评分量表)。结果:各组临床症状及日常生活能力与治疗前均有了明显的改善;通心络组临床疗效优于常规治疗组,差别均有统计学意义(P<0.05)。而各中医证候通心络各组之间无明显差异,提示不同中医证候脑梗死患者中,通心络胶囊对其作用无明显差异。结论:通心络胶囊能减少脑梗死患者神经功能异常,促进患者的康复,但在不同中医证候患者中作用无明显差异。

Objectives： To observe the effect of Tongxinluo Capsule on prevention of two cerebral infarction in different traditional Chinese medicine（（TCM） syndromes. Methods： In 200 cases of cerebral infarction patients,the cases of TCM diagnosis of qi deficiency and blood stasis,kidney yin deficiency syndrome,wind and phlegm,fire syndrome group was 50 cases. They were randomly selected10 patients with different TCM Syndromes as routine treatment group. The 40 cases were received routine inhibition of platelet aggregation and lipid therapy. The rest 160 patients were treated with routine treatment and Tongxinluo capsule treatment. After 30 days,the patients were observed the improvement of clinical symptoms. The symptoms score were recorded according to the National Institutes of Health Stroke Scale（NIHSS scale）,modified Rankin Scale（mRS scale） and daily life ability scale（BI scale）. Results： Compared with before treatment,Every groups have significant curative effect. Compared with routine group,the clinical effect of patients with Tongxinluo group were better than the routine group,and there is significant differences between them（P 0. 05）. But there is no obvious difference in four Tongxinluo groups. It suggests that Tongxinluo has the same effect in different TCM syndromes. Conclusion：Tongxinluo capsule can reduce the abnormal nerve function,promote the rehabilitation of patients,it has no significant difference in different TCM Syndromes.
