腹部CT诊断胃十二指肠溃疡急性穿孔的价值探讨

Discussion on the value of abdominal CT in the diagnosis of acute perforation of gastroduodenal ulcer

目的:探讨、分析腹部CT诊断胃十二指肠溃疡急性穿孔的价值。方法:收治胃十二指肠溃疡急性穿孔患者50例,均接受腹部平片及腹部CT诊断,并经手术病理确诊。回顾胃十二指肠溃疡急性穿孔患者的CT资料,分析其诊断价值。结果:腹部CT平扫检查可见腹内游离气体46例(92.00%);腹部立位平片检查出膈下游离气体35例(70.00%),腹部CT的检出率显著更高(P<0.05);穿孔位置可见小气泡、渗出液41例(82.00%),腹腔积液12例(24.00%),胰腺假性囊肿1例。结论:临床对胃十二指肠溃疡急性穿孔患者进行诊断可采用腹部CT,能快速准确地为诊疗提供有利依据。

Objective:To investigate the value of abdominal CT in the diagnosis of acute perforation of gastroduodenal ulcer.Methods:50 patients with acute perforation of gastroduodenal ulcer were selected.All patients received abdominal plain film and abdominal CT diagnosis.The diagnosis was confirmed by operation and pathology.We reviewed the CT data of patients with acute perforation of gastroduodenal ulcer and analyzed its diagnostic value.Results:Abdominal CT scan showed 46 cases of intra-abdominal free gas(92%).35 cases(70%)of diaphragm gas were examined by abdominal vertical radiograph,and the detection rate of abdominal CT was significantly higher(P<0.05).Perforation showed small bubbles,exudate in 41 cases(82.00%),ascites in 12 cases(24.00%),pancreatic pseudocyst in 1 case.Conclusion:Abdominal CT can be used in the diagnosis of acute perforation of gastroduodenal ulcer,and it can provide a favorable basis for the diagnosis and treatment of acute perforation.
