N端脑利钠肽前体、降钙素原和超敏C反应蛋白在慢性阻塞性肺疾病急性发作期患者血清中的表达及临床意义

Expression and clinical significance of N terminal brain natriuretic peptide precursor, calcitonin and hypersensitive C reactive protein in the serum of patients with acute exacerbation of chronic obstructive pulmonary disease

目的:观察N端脑利钠肽前体(NT-Pro BNP)、降钙素原(PCT)和超敏C反应蛋白(hs-CRP)在慢性阻塞性肺疾病急性发作期(AECOPD)患者血清中的表达。方法:收治AECOPD患者80例作为观察组,同期收治COPD稳定期患者60例作为对照组,比较相关指标。结果:观察组治疗前血清NT-pro BNP、PCT和hs-CRP表达水平明显高于对照组(P<0.05);观察组治疗后上述指标明显低于治疗前(P<0.05);观察组不同肺功能分级患者血清NT-pro BNP、PCT和hs-CRP表达水平差异均有统计学意义(P<0.05),随着肺功能分级升高,血清NT-pro BNP、PCT和hs-CRP表达水平呈逐渐升高趋势(P<0.05);好转组患者血清NT-pro BNP、PCT和hs-CRP表达水平均明显低于死亡组(P<0.05)。结论:血清NT-pro BNP、PCT和hs-CRP可作为AECOPD患者病情评估、预后判断、指导临床治疗的敏感性实验室指标。

Objective:To observe the expression of N terminal pro brain natriuretic peptide(NT-Pro BNP),procalcitonin(PCT)and high sensitivity C reactive protein(hs-CRP)in serum of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:80 cases of AECOPD patients were selected as the observation group,in the same period,60 patients with COPD stable period were selected as the control group,we compared the relevant indicators.Results:The expression level of serum NT-pro BNP,PCT and hs-CRP in the observation group was significantly higher than that of the control group before treatment(P<0.05).The above indicators in the observation group were significantly lower than those before treatment(P<0.05).The difference of serum NT-pro BNP,PCT and hs-CRP levels between the two groups was statistically significant(P<0.05),the levels of NT-pro BNP,PCT and hs-CRP in serum increased gradually with the increase of pulmonary function(P<0.05).The serum levels of NT-pro BNP,PCT and hs-CRP in patients with improvement were significantly lower than those in the death group(P<0.05).Conclusion:Serum NT-pro BNP,PCT and hs-CRP can be used as a sensitive laboratory index for evaluating the condition of AECOPD,judging the prognosis and guiding the clinical treatment.
