辨证施护在小儿幼年特发性关节炎(JIA)中的临床效果观察

Clinical Curative Observation of Dialectical Nursing for Children with Juvenile Idiopathic Arthritis

目的:观察辨证施护在小儿幼年特发性关节炎中的临床效果。方法:选择2014年1月至2016年12月之间接受治疗的幼年特发性关节炎患儿86例。根据数字表法随机分为实验组和对照组,每组各43例,两组患儿均给予中药自拟通络汤剂治疗,对照组采取常规的中医护理,实验组在对照组的基础上,采取中医辨证施护措施。对两组患儿的治疗疗效、血清炎症因子水平、生活质量和对护理满意度比较分析。结果:实验组的治疗有效率为93.02%,对照组为83.72%,实验组显著高于对照组(P<0.05)。护理前,两组患儿血清炎症因子水平无统计学意义(P>0.05),护理后,其炎症因子水平均下降,组间比较,实验组改善程度显著高于对照组(P<0.05)。护理后,对照组的生活质量评分显著低于实验组(P<0.05),社会方面差异不显著。实验组护理满意度显著高于对照组,具有统计学意义(P<0.05)。结论:辨证施护在小儿幼年特发性关节炎的治疗中,可以降低其炎症因子水平,改善生活质量和促进早日康复,值得临床推广。

Objective： To investigate the clinical effect of dialectical nursing in juvenile idiopathic arthritis. Methods： 86children with juvenile idiopathic arthritis treated between January 2014and December 2016were selected. According to randomly divided into experimental group and controlled group, 43cases in each group, two groups were given Chinese herbal medicine Self-made Tongluo Decoction, the controlled group took Chinese medicine nursing, the experimental group took TCM dialectical nursing measures on the basis of the controlled group. The curative effect, serum inflammatory factors, quality of life and satis- faction with nursing were compared between the two groups. Results ： The effective rate of the treatment group was 93.02% , the controlled group was 83.72%, and the experimental group was significantly higher than that of the controlled group （ P〈0. 05 ）. Before the nursing care, the serum inflammatory factors in the two groups were not statistically significant （ P〉0. 05 ）. After the nursing, the levels of inflammatory factors decreased, and the improvement in the experimental group was significantly higher than that in the controlled group （ P〈0. 05 ）. After nursing, the quality of life score in the controlled group was significantly lower than that in the experimental group （P〈0. 05 ）, but there was no significant difference in social dimensions. The nursing satisfaction of the experimental group was significantly higher than that of the controlled group, with statistical significance （P〈 0. 05 ）. Conclusion ： In the treatment of juvenile idiopathic arthritis, it can reduce the level of inflammatory factors, improve the quality of life and promote early rehabilitation. It is worthy of clinical promotion.
